II. FAMILY COMPOSITION


Saturnino Rabida : 55 years old husband,elementary
undergraduate,skilled worker( masonry)
Zyzill Rabida : 4 years old grand daughter

III. PROBLEM PRESENTED


Client seek possible medical assistance for her possible blindness problem.
FINDINGS
Walk-in clients is a mother to four children.A 49 years old mother from Barangay Mabini,San
Fernando,Romblon found to have CATARACT and GLAUCOMA.
According to patient it started in her left eye blurredness until it become totally dark in
1991.Because of financially incapacitated and children were all at school,she refuse to consult the
doctor.It affects the right eye of the patient that nearly lost her vision.
She was convinced by her husband to have medical consultation now that all children were
married.Last February 9,2019 they travel to Manila for medical check-up.They consulted at
LACSAMANA EYE CLINIC and was diagnosed of having cataract and glaucoma and was advice
to continue medication.
According to client,she had early consultation last January 10,2019 at IEC International Eye
Center and was diagnosed of having cataract and glaucoma on both eyes.On January 31,2019 she
underwent cataract extraction with PCIOL implantation on her right eye.On the same day a
Trabeculectomy Surgery was also done.
Because of financial constraint, the patient’s medical prescription was not followed.Only those
vitamins that cost less were taken.The 55 years old husband still trying to comply all the medical
needs but being jobless is hard to provide the needed prescription from the attending physician.
An estimated Php100,000.00 for her six months treatment triggered the couple on how to
provide.A PWD member of the society,the client is also a member of the indigent family of her
barangay.

IV. ACTION TAKEN
1. Office interview
2. Made Social Case Study Report;
3. Provide medical assistance
V. RECOMMENDATION
Based on the above findings, client is very much illegible to avail of medical / financial
assistance , therefore recommended by the undersigned worker in the amount of Five thousand pesos
(Php 5,000.00) charge to MSWDO Budget.
