ABOUT ACCIDENT
Occurred at 4:37 AM, March 28th 1979. Operated valves in unit 2
closed due to a malfunction and caused a shut off of the supply
water to primary feed water system.

The temperature rises until the reactor core completely melted and
radioactive gas emitted to the environment

Workers had not prepared in any of their training and were forced
to make decision on the spot
-
A rise in temperature led the reactor to go automatic shut down for
one second. It -took them hours to realize the valve had not closed a
it should have -ten seconds after it first opened
-
This has caused- a coolant water to leak, and high pressure injection
pumps automatically pushed water into the reactor system to
compensate for the lost pressure
THE ROOT CAUSE
At 6:22 AM, the operators closed the valve connecting the relief
valve and the pressurizer. This prevented coolant water from leakin
The accident began with failures in the secondary loop that out further, but the overheated steam that was created blocked
resulted in a loss of feed water to the secondary side of the water from reaching the cooling system
Of steam generators
heat and pressure increased in the closed primary loop
additional failure of a primary pressure relief valve
magnified the problem.
Operators unaware valve was open

ACTION HAD BEEN TAKEN

school were closed and residents were urged to

stay indoor
Governor advised to evacuate the pregnant
women and children within 5 miles
extended the evacuation zone to 20 miles
start cleanup process in August 1979 until
December 1993
