Basingstoke train approaching CJ
Signal faulty when the driver saw the signal shift
Inadequate training, from green to red.Incapable of
assessment, supervision & stopping, he stopped the train at the
testing next signal & then telephoned to the
Overworked & under supervised signal box.

DEATH signal shift from green to red.
Incapable of stopping, he
stopped the train at the next
signal & then telephoned to the
69 signal box.
SERIOUS Shortly after, a 2nd train from
INJURIES Bournemouth collided with the
Basingstoke train.
A 3rd train in the other
direction on the adjacent line
415 & collided with the wreckage
MINOR just after the initial impact.
INJURIES The 4th train driver saw the
other train & managed to come
to a stop behind the other 2.

CAUSES OF ACCIDENT

Signal faulty
Inadequate training,

ACTIONS THAT HAD BEEN TAKEN

Unused signal wires had been cut & insulated. Testing are
made by an independent person
Signal technician attend refresher courses every 5 years &
tester are trained & certified
Management ensured that no one is over-worked & senior
project manager are responsible for all aspect of the project
Unprotected wrong side signal failures are immediately
reported to Railway Inspectorate
Cab radios, begin installing public address system on existing
trains that were not expected to be withdrawn within five
years
Testing was mandated & working hours for employees that
involved in safety critical work were limited
Introducing automatic train protection system, cab radios &
black boxes
British Rail commissioned a major safety audit & inspecting
every comparable piece of technology

Ensure that installation and


testing work is carried out at a
much higher standard.
Improved communication must
exist between all levels of
organisation.
