Signal faulty

ACTIONS THAT HAD BEEN TAKEN
Unused signal wires had been cut & insulated. Testing are
made by an independent person
Signal technician attend refresher courses every 5 years &
tester are trained & certified
Management ensured that no one is over-worked & senior
project manager are responsible for all aspect of the project
Unprotected wrong side signal failures are immediately
reported to Railway Inspectorate
Cab radios, begin installing public address system on existing
trains that were not expected to be withdrawn within five
years
Testing was mandated & working hours for employees that
involved in safety critical work were limited
Introducing automatic train protection system, cab radios &
black boxes
British Rail commissioned a major safety audit & inspecting
every comparable piece of technology
