The following questionnaire is designed to evaluate the effectiveness of


the Supervised Industry Training (SIT) or On the Job Training (OJT) you had
with the Industry Partners of Paliparan National High School in Paliparan
III, Dasmariñas City, Cavite. Please check (/) the appropriate box

evaluation shall serve as a basis for improving the design and management
of the SIT in Paliparan National High School to maximize the benefits of the

General Interpretation :
The trainers were equipped with knowledge and skills they needed to become
efficient and effective trainers for the qualification EIM NC II. The workshop
layout does not conforms with the components of a CBT workshop because there
are equipment which are not intended for the course EIM NC II.

Recommendation:
To be more effective and efficient, the trainers should enhance their skills and
knowledge , they should be given more training related to their course. Remove
all equipment that are not related to the course and provide more area/cabinets
for the tools and equipment.

1. Given the necessary tools, materials and equipment, you must be able to
(Allotted time: 5 hrs.)

 Prepare electrical power and hydraulic tools

2. Assessment shall be based on the units of competency in the Training


Regulations and Evidence Plan and shall focus on the evidence gathering
