1.0 INTRODUCTION

1.1 General

PT. Bakrie Construction recognizes the importance of planning Health and Safety into the
project at the earliest possible stage to ensure that injuries and ill health, arising from the
construction activities are to be avoided. HSE management plan has been produced to describe
how PT. Bakrie Construction intends to manage, control and coordinate the work. The
implementation of this HSE shall ensure the Health & Safety of all persons engaged on the
project.

PT. Bakrie Construction acknowledges the importance placed upon Health, Safety and
Environment matters and shall provide full cooperation and assistance with the client or
Company.

For this HSE Management Plan to be effective it is essential that the management and
supervising staff of PT. Bakrie Construction are familiar with and implement all the
requirements of the plan. PT. Bakrie Construction Management, supervising personal and client
will therefore be provided with a controlled copy of the plan.

PT. Bakrie Construction will actively seek the support of all personnel engaged on the project
to look after the Health, Safety and Environment of themselves and their colleagues by working
within the spirit and intent of the HSE Management Plan. All PT. Bakrie Construction
personnel are required to know the contents of the HSE Management Plan.

Any person found to be disregarding the requirements of the HSE Management Plan Should be
the subject of disciplinary action, which may result in their dismissal from the project.

2.0 PURPOSE

To ensure the Emergency Response Plan is available and implemented if any Emergency situation
such as fire & explosion, rupture, illness and injury or death, property damage during project activity
at Yard.
The emergency preparedness and response GOAL is to reduce life, property and economic losses

3.0 SCOPE

This Emergency Response procedure covers the program to reduce or eliminate risk of injury or death
to employees and / or damage or loss of property and management of personnel evacuation should it
be required from the yard to the designated clinic and hospital in Cilegon. Evacuation will be required
following accidental events such as:

 Accident involving personnel with or without injury


 Accident resulting in personnel fatality

 Fire on work location, office, accommodation, and the like.


 Heavy equipments and Car Accident/Traffic accident

5.0 AUTHORITY

The General Manager Yard & Facilities, Fabrication Manager, Maintenance Yard & Facilities
Manager, Yard Support manager, HSE Manager, HSE Superintendent, Superintendent and Supervisor
are responsible to ensure that all personnel at yard are aware of proper and response to procedure and
evacuation in any emergency.

6.0 TYPE OF EMERGENCY

6.1 Moderate

Incase of accident on site Person train in first aid (Superintendent, Supervisor, HSE
Superintendent and Safety Officer/first aider) treats at yard and evacuate the injury to the clinic
and after treatment return to work.

6.2 Serious

In case of accident General Manager Yard & Facilities, Fabrication Manager, Maintenance
Yard & Facilities Manager, Yard Support Manager, HSE Manager, Superintendent, Supervisor
HSE Superintendent, Safety Officer and First Aider are responsible to evacuate the injury to the
clinic at yard immediately or call the clinic by radio channel or telephone, asking to doctor or
paramedic on duty to come on incident location. General Manager Yard & Facilities and HSE
Manager and Doctor have to decide whether the injury needs to be evacuated to hospital

6.3 Fatality

Do not move anything until the police arrive on accident location. Evacuate the body
immediately after having permission from the police to nominated hospital by special boat

7.0 RESPONSIBILITY

7.1 Fabrication Manager, Superintendent & Supervisor

In the event of accident (as described under II. Scope) the Fabrication Manager, superintendent/
supervisor shall stop the work and inform or contact immediately General Manager Yard &
Facilities, HSE Manager, HSE Superintendent on duty to ask for the emergency team, fire team
or doctor/medic to come to the location immediately.

7.1.1 Report all detail of incident occurrence:

 Location of the incident.


 Name of injury.
 Brief details of injured and patient condition.
 Description of accidental event (accident, fire, explosion. etc…)
 Advice of any special condition
 Time of accident occurrence.

7.1.2. Ensure the area of accident, fire or explosion is isolated so it will be accessible for
fire/Evacuation in this location.

7.1.3. Doctor/Paramedic/trained person shall try to keep talking with the injured to prevent him
from unconscious

7.1.4. Except of doctor/paramedic or trained person no body is allowed to handle the injured.

7.2 HSE Superintendent

1. In case of emergency cases collect information from competent sources.

2. Report immediately to HSE Manager and Fabrication Manager the detail information include
the injury condition and action that has been taken.

3. Ensure the first aid given by first aider or paramedic on duty

4. Ensure the location is isolated with barricade so it will be accessible for fire/Evacuation team

7.3 HSE Manager

1. Observe and collect any information from competent sources

2. Inform the emergency response team or doctor/medic in the clinic regarding the accident,
between accident and two way communication on Radio Channel and telephone Number
(must be established permanently) to be ready for further handling

3. Inform to General Manager Yard & Facility and President Director the detail information
immediately, report the injury condition and action that has been taken.

7.4 Yard Support Manager

1. Coordinate with HSE Manager to have access of facilitation needed to evacuate the
injured to hospital.

2. Inform to General Manager Yard & facilities immediately regarding the situation and
handling of injury. Preliminary shall be raised in 12 hours.

3. Inform the local authority such as Police relating the accident.

4. In case of fire he will act as On Scene Commander (OSC) and together with HSE Manager
he will give necessary direction to his subordinates for fire fighting and evacuation.

5. Notify the family of the injured and explain the action had been taken.

7.5 General manager Yard & Facilities

Ensure that the mechanism of handling person and property are properly manner.

7.6 HRD/ ADM

(Human Resources Department/Administration)

The HRD shall make an arrangement to the appointed hospital.

If the injury has to be hospitalized in Jakarta, the HRD/ADM shall make an arrangement to
recommended hospital in Jakarta

1. To ensure the driver knows the rout from the Yard Sumuranja and destination.

2. Appoint one person to support the evacuation to anticipate any question about the chronology
of incident.

3. Inform the police or other local authorize for fire and fatality cases

8.0 YARD EMERGENCY PROCEDURE

In case of Emergency at yard the General manager Yard & Facilities, Fabrication manage,
Maintenance Yard & Facilities Manager, Yard Support Manager, HSE Manager, HSE Superintendent
and Superintendent will take action immediately:

 Stop all of activities immediately

 Inform to all the people that they will escape to the nearest muster point and stand by until
further notice by authorize person.

 Count all the personnel at muster point according to the POB list.

 HSE Superintendent will inform the HSE Manager as a detail concerning the incident (fire &
explosion, rupture, illness and injury or death, property damage).

 Evacuate all personnel to Muster Point or other safe area immediately and wait for further
instruction from the authorized officer.

 Evacuate all personnel at muster point to safe area immediately and wait until the situation is
clear as needed.

9.0 EVACUATION PROCEDURE FROM WORK LOCATION TO YARD CLINIC

In case of incident at work location, the medic in charge at location will give first aid and evacuated
to yard clinic for doctor observation.

9.1 Action to be Taken For Evacuation

a. The medic in charge on the work location must be completed with radio handy talky and
first aid kit bag (Emergency bag)

b. In-case of any emergency information through the radio the medic has clear receive
information then go to the incident location immediately completed with the emergency
bag and stretcher

c. Emergency team must assist the medic to give first aid to the injured.

d. The medic has to decide that the injured need to be evacuated to the clinic at yard or not
for doctor examination.

e. if yes the medic has to inform to HSE Superintendent on duty to notify the doctor in the
clinic that the injured will be evacuated to the clinic for doctor examination

f. During evacuation the injured to the yard clinic the medic has to assist the injured

10.0 EVACUATION PROCEDURE FROM WORK LOCATION TO CILEGON


HOSPITAL

In case of incident at yard and to be evacuated to Cilegon Hospital.

10.1 Action to be Taken For Evacuation

1. The Doctor has to decide that the injured will evacuated to the Cilegon Hospital or not

2. if yes the doctor has to inform to HSE Manager for Ambulance arranging from the Yard to
Cilegon Hospital

3. Doctor has to inform Cilegon Hospital that the injured is on the way to the hospital.

4. During evacuation from yard clinic to Cilegon hospital the medic has to assist the injured

11.0 EMERGENCY COMMUNICATION

Refer to the HSE Communication Procedure (BCPRD-HSE-P44_00)

12.0 YARD EMERGENCY RESPONSE TEAM

1. Fabrication Manager

3. HSE Superintendent
4. Superintendent
5. Doctor Stand by in the clinic
6. Medic each work location
7. 20(twenty) trained people in First Aid, Fire fighting

13.0 EMERGENCY RESPONSE PROCEDURE TESTING AND EVACUATION

To ensure the Emergency Response Plan is working, the regular monthly Emergency drill will be
routinely conducted as per schedule and follow with Emergency Drill Evaluation. In every
Emergency Drill and drill evaluation all personnel are actively participated and also at the same time

all the Alarms are tested for alarm familiarization. Inputs and correction during drill evaluation is
very important in maintaining the assigned Emergency Response Team ready at any emergency
situation.

14.0 EMERGENCY DRIVES

Emergency devices available at yard

14.1 Yard Area

1. Fire and Alarm system at yard

2. Smoke detector in main office

3. Portable fire extinguisher at yard, workshop, Chamber, Main office, Warehouse, Yard office
and Client office

15.0 FIRE/ EXPLOSION

In the event of fire / Explosion at yard the following action shall be taken:

1. Stop the work. Switch off all the power or fuel sources.

2. Do not panic. Follow the ERT Leader instruction (HSE Superintendent) to reach the Muster
Point along the escape way.

3. Alert all people for the fire hazard and communicate with area authority to activate
fire/explosion alarm

4. Bring out the First Aid Kit and importance software/copies of important file of the management

5. Extinguish the fire using any available fire fighting equipments while the fire is still small, but
leave it when the fire gets bigger or uncontrollable.

6. Superintendent, Supervisor, shall report to Fabrication manager immediately of the fire /


explosion case as detail by two-way communication or talk to them directly.

7. HSE Manager shall contact the HSE Superintendent by two way communications immediately
to cover the situation and to avoid severity and or mitigation.

8. HSE Manager shall notify the fire / explosion incident to General Manager yard & Facilities by
radio or telephone. Standard statement needed for reporting is as below.

9. If the fire can not be controlled locally inform the surrounding company fire brigade for further
assistant

10. Carry out the investigation by the team and report to President Director in 12 hours (preliminary
report)

16.0 SMALL FIRE

1. Alert the people in the vicinity of fire.

2. Extinguish the fire by using the correct fire extinguisher by trained people.

3. Report the occurrence to the Fabrication Manager, HSE Manager, Superintendent or HSE
Superintendent on duty.

“ALL FIRE (No Matter How Small) MUST BE REPORTED TO COMPANY SITE
REPRESENTATIVE ON DUTY IMMEDIATELY”

17.0 OIL SPILLAGE

 All persons found the emergency situation (chemical leaks, fuel/oil spill) shall report to
Fabrication Manager, Maintenance Yard & facilities Manager, HSE Manager, Superintendent
or HSE Superintendent.

 Any leak of oil, gas and spillage which flows to the sea shall inform to Fabrication Manager,
Maintenance Yard & Facilities Manager, HSE Manager, HSE Superintendent or
Superintendent.

 Notify the emergency response team to attend the scene for clean up.

 Assess the situation and ensure that the steps were taken to stop the leak or spillage or inform to
Maintenance Yard & Facilities Manager that the leak point /the drainage point to be plugged
and isolate the spillage.

 Announce all the workers and inform them concerning of the condition and situation.

 Stop all activities and cancel all relevant work permits.

 Consider the suspension of any activities, which may be affected by the leak or spillage

 The emergency response team members shall assist in cleaning up the leakage or spillage on
the sea. Where possible the spillage liquid should be collected.

 If fuel, oil have been spillage into the Yard ground or sea, inform immediately to Fabrication
Manager, Maintenance Yard & Facilities Manager, HSE Manager, HSE Superintendent or
Superintendent that the decision must be taken to solve the oil or fuel spillage on the sea or
Yard

18.0 GAS LEAK

In case of gas leak from the main tank the Maintenance Yard & Facilities Manager, HSE Manager,
HSE Superintendent and Superintendent will take action immediately:

 Stop all of activities and switch off all ignition sources immediately

 Inform to all the people escape or evacuated to the nearest muster point and stand by until
further notice by authorize person.

 Count al the personnel at muster point according to the POB list.

 Superintendent and HSE Superintendent will inform the Fabrication Manager, Maintenance
Yard & Facilities Manager, or HSE Manager as a detail concerning the gas leak from the main
tank.

In Case the gas leak at the bottle in used that the person knows of this case that have to
report to Supervisor, Superintendent, HSE Superintendent, Safety Officer in-charge then he/
they will take action to stop the leak immediately.

19.0 GAS LEAK/ FIRE/ EXPLOSION AT STORAGE

In case of gas leak/fire/explosion at storage the Fabrication Manager, Maintenance Yard & Facilities
Manager, HSE Manager, HSE Superintendent and Superintendent will take action immediately:

 Stop all of activities and switch off all ignition sources immediately

fire/explosion location and stand by until further notice by authorize person.

 Count al the personnel at muster point at the yard according to the POB list.

 HSE Superintendent and Superintendent will inform the Fabrication Manager, or HSE Manager
as a detail concerning the above incident.
