Conducting Fatality/Dismemberment Investigations

1. All fatal accidents or those resulting in dismemberment should be reported to the


regulatory agency immediately and a written report subsequently submitted with
no delay

2. Officers with the responsibility for monitoring safety programmes and ensuring
compliance should visit and photograph fatal accident scenes within twenty-four
hours of the incoming accident report

3. A preliminary investigation should be conducted within twenty-four (24) hours of


the incoming accident report

4. A preliminary report should be generated from the preliminary investigation,


which should be able to perform the following:

a. Describe the incident location


b. Describe/reconstruct the incident scene
c. Describe the incident day (weather, sun position, etc.)
d. State the incident time
e. State the involved employees and their apparent state due to the accident
f. State the witnesses (primary, secondary & tertiary) and the type of
information that can be obtained from them
g. Give an overview of the chain of events leading up to the accident
h. Highlight possible accident causes

5. The preliminary report as well as the initial submitted written report from the
involved company should be analysed to generate and compile questions to garner
the required additional information regarding the accident

6. A formal investigation should be conducted within fourteen (14) days of the


incoming report re the accident, during which all relevant individuals should be
interviewed/interrogated

7. An investigation report should be generated from the formal investigation and


should be able to perform the following:

a. Describe the investigation process and team


b. Describe the incident location
c. Give a detailed description of the incident scene
d. Describe the incident day and its contribution to the accident
e. State the incident time and its contribution to the accident
f. Describe in details the contribution of the involved employees to the
incident
g. State conclusively the effects of the accident on involved employees
h. Give detailed witness reports highlighting relevant information
i. State additional facts pertaining to the accident including management
systems
j. Analyse the garnered information and state the analytical method used
k. State the conclusions of the investigative team regarding the causal factors
(root and contributory) – system or otherwise
l. Recommend appropriate corrective measures

8. The investigation report should be generated and submitted for viewing within
forty-five (45) days of the incoming report

9. An enquiry into the incident, hosted and led by the monitoring agency, should be
convened within ninety (90) days of the fatal accident

10. The recommendations for corrective measures should be generated within


fourteen (14) days of the enquiry and should incorporate those from the formal
investigation

11. The recommendations for corrective measure should be communicated to the


relevant organization within fourteen (14) days of the enquiry
Conducting Lost Time Investigations

1. All lost time accidents should be reported to the regulatory agency and a written

four (24) hours of the incoming accident report

3. A preliminary investigation should be conducted within five (5) days of the


incoming accident report

4. A preliminary report should be generated from the preliminary investigation,


which should be able to perform the following:

a. Describe the incident location


b. Describe/reconstruct the incident scene
c. Describe the incident day (weather, sun position, etc.)
d. State the incident time
e. State the involved employees and their apparent state due to the accident
