I, JOHN MARK P. SUAL, 19 years of age, a 4 TH year student of Bachelor of Secondary

Education Major in English, a Filipino citizen and a resident of P-2 Caramcam District
Mangagoy, Bislig City, after having been sworn, hereby depose and say:

1. That I am selected as the President of the Bislig Campus Student Council (BCSC) in
USeP-Bislig Campus;

2. That as such, I am responsible for the safety and welfare of officers and the
members including those who are applying for membership of the said
organization;

3. That I shall be responsible for whatever activities shall undertake.

4. That I am executing this affidavit for the legal intents and purpose.
