Congratulations on your purchase of LG Airconditioner that comes with the promise of great quality and
performance. We are pleased to inform that with the Airconditioner with model number JW-Q18WUZA you
have purchased you are entitled for a COMPLIMENTARY INSURANCE* for 1 year against Novel Corona Virus
(nCoV), (COVID-19) for Rs. 30000 from Cholamandalam MS General Insurance Co. Ltd. that will be valid from
the date of purchase as per the original invoice, under the policy number 2893/10184943/000/000.

As the world is witnessing a current global crisis of Corona Virus pandemic, it is a small gesture from us at LG,
as token of thanks for your continued patronage and support and making us you’re most preferred choice.

The coverage under insurance will be as under:

Insurance Coverage:
If at any time during the policy period, the Insured Person contracts and is diagnosed with Novel Corona Virus
(nCoV), (COVID-19), the Insurer shall pay a Lump Sum benefit equal to 100% of the Sum Insured.

Specific Conditions applicable to the coverage:


1. Payment will be made only on Positive diagnosis for Novel Corona Virus (nCoV) (COVID-19 )
2. The diagnosis of the covered condition has to be done by authorized centres as declared by the Union
Health Ministry of India
3. Diagnostic Test means Real time PCR assay for RNA virus done by authorized centres as declared by
the Union Health Ministry of India.
4. This is onetime benefit applicable for the entire tenure of the Policy and shall terminate upon
payment of this benefit;

Waiting Period:
The Insurer shall not be liable for any claim arising for Novel Corona Virus (n CoV) (COVID-19) within 16 days
from the first policy commencement date with them.

General Exclusions applicable under the policy:


The policy does not cover any losses /claims caused due to the following:
1. Any treatment taken for any disease other than Novel Corona Virus (n CoV)(COVID-19)
2. Any condition for which the insured had signs or symptoms, and/or was diagnosed, and/or received
medical advice/treatment within the waiting period.
3. Testing done in a centre other than the ones authorized in India
4. Diagnosis and treatment outside India.
5. Any treatment taken on OPD basis
6. If the Insured has travelled outside India within last 3 months.
7. Treatment in any hospital or any other provider network that we blacklisted as listed in our website
www.cholainsurance.com.However , this exclusion will not apply in case of emergency hospitalisation
,subject to verification of claim.

Claims Procedure:
Notification: Claim notification to be given to Chola MS immediately on occurrence of the event and
not later than 7 days from the date of Diagnosis by telephone through toll free number (1800-208-
5544) or in writing by email (customercare@cholams.murugappa.com) / letter). The intimation
should contain the following information:
Insured details (Name /Age/Gender)
Contact no & E-Mail ID
Policy Number
Date of Diagnosis
Claim Submission Procedure: Completed and duly signed claim form with the following documents to be
submitted within 15 days from the date of diagnosis/discharge to CholaMS Claims Office, whose address is
given below
Claim Documentation*:
a) Govt. issued ID proof.
b) Original Purchase invoice of LG product on which this coverage is applicable (subject to data logged in
the portal provided by LG
c) Detailed discharge summary (if hospitalized) /Death summary (if hospitalized) specifying the disease &
treatment details.
d) COVID - 19 confirmatory lab report from Govt. or authorized center as declared by Govt. of India.
e) KYC of the nominee / legal heir in case of death claim and KYC of the Insured for other claim under the
policy.
f) Account details with proof for NEFT of the nominee / legal heir in case of death claim and of the
insured for other claims under the policy i.e. cancelled cheque, passbook
Delay in intimation of claim
It is essential and imperative that any loss or claim under the policy has to be intimated to us strictly as per the
policy conditions to enable us to appoint investigator wherever deemed necessary for loss assessment. This will
enable us to render prompt service by way of quick and fair settlement of claim, which is our primary motto.

Any genuine delay, beyond the Insured’s control will definitely not be a sole cause for rejection of the claim.
However any undue delay which could have otherwise been avoided at Insured Person’s end and especially if the
delay has hindered conducting investigation on time to make proper assessment, to mitigate further loss, if any
may not only delay the claim settlement but also may result in claim getting rejected on merits.

Coverage Conditions

The coverage to the buyer will be for 12 months from the date of purchase invoice of the designated
product under the scheme being offered by LG Electronics
The coverage under the policy will be strictly to the person whose name is appearing in the purchase
invoice subject to the product being purchased is covered under the scheme offered by LG and has
been agreed by CholaMS for the coverage under the policy .
The purchase invoices has to be on the name of an individual person, in case of purchase invoice is
issued in any entity’s name , the coverage will be excluded from the policy against that said invoice
Each and every buyer (for the decided product) has to mandatorily register on the portal /link
provided by LG immediately on purchase of the designated product for the Coverage to be effective,
The purchase invoice date necessarily should be between 12 months of the master policy issued to
LGEIL by CholaMS. LGEIL has the right to foreclose the scheme before the policy end date,
Invoices has to be named and dated and invoice is not transferable and the scheme is not applicable
on secondary/re-sale
The invoice has to be of a single product only, separate invoices to be issued if someone is purchasing
multiple product.
The insurance cover is limited to single policy per person irrespective of number of invoices, more
than one i.e. two or multiple invoices in a single name cannot make the buyer entitle for multiple
coverage under the policy.
Age of the buyer between 03 months to 65 Years max.
