That I, ANA LUZ J. HALICAN, of legal age, Filipino, widow and a resident of
Greenfields Subd. Brgy. Cubay, Jaro, Iloilo City, do hereby name, constitute, and
appoint, EUGENIO F. JALANDONI of legal age, Filipino, single and a resident of
Greenfields Subd. Brgy. Cubay, Jaro, Iloilo City, to be my true and lawful attorney-in-
fact, for me and in my name, place and stead, to do and perform the following acts and
things, namely:

1. To claim and activate my ATM Card to be issued by Union Bank J.M

Basa Branch

2. To withdraw over the counter my monthly personal and survivorship

pension before the above-mentioned Bank;

3. To do and perform such act and undertaking as maybe necessary to

give full force to the foregoing authority

HEREBY GIVING AND GRANTING unto my said Attorney-in-Fact full power

and authority to do and perform each and every act which may be necessary or
convenient, in connection with any of the foregoing as fully to all intents and purposes
as I might or could do, if personally present and acting in person, HEREBY RATIFYING

under and by virtue of these presents.

AFFIDAVIT OF SUPPLEMENTAL REPORT

I, LEOTINA MALLARE NIELO, Filipino, of legal age, married, and resident of

Brgy. Yulo Drive, Arevalo, Iloilo City, Philippines, having been duly sworn in
accoradance with law, hereby depose and state:

1. That I was born on June 22, 1966 at Baguio City to the Spouses Gil S. Mallare
Jr. and Cristina Gaceta Aremin;

2. That since I was young, I have always been known and called by the name
“LEOTINA” and that it is the same first name that I have been using in all
my documents and identification papers;

3. However, it appears from the records of the Office of the Civil Registrar of
Baguio City that I was registered therein as “BABY GIRL” as my given first
name;

4. That it is my desire to be registered with the said Office of the Civil Registrar
of Baguio City with the first name “LEOTINA”;

5. That the failure to state my first name was pgurely inadvertence of my

parents who failed to do so because of their uncertainty at first as to what
name would be given me at the time of the registration of the facts of my
birth with the said office;

As such, I execute this Affidavit to certify or attest to the truth of the foregoing
facts and for whatever legal purpose that this affidavit may serve.
