1.0 Introduction

This HSE plan covers the incident prevention programs to be employed during
the CONSTRUCTION, REPLACEMENT & REPAIRS OF FLOWLINES IN SPDC
- LAND. All employees shall abide by all the safety rules in this plan including
those that shall be prescribed by Shell Petroleum Development Company
(SPDC).

 INDEXPRO INT’L SERVICESLIMITED HSE Policy


INDEXPRO believes that NO JOB or TASK IS MORE IMPORTANT
THAN WORKERS’ HEALTH AND SAFETY. If a job represents a
potential safety, health or environmental threat, every effort shall be
made to plan a safe way to do the job. Every procedure must be a safe
procedure. Shortcuts (cutting corners) as safe procedures by workers
shall not be tolerated. If a worker observes any job, which may pose a
potential threat to their health or safety, he or she must inform
INDEXPRO management and management shall take adequate
precautions/control and recovery measures if control fails.

 Project HSE Objective


INDEXPRO plans to execute this project/contract to acheive goal zero, i.e
without any injury to its personnel, Damage to equipment, Impact/damage to the
environment by doing the following:
Assessing all risks and putting appropriate controls and recovery measures in
place.
 Using competent/qualified personnel.
 Using standard and fit-for purpose equipment & tools
(Premobbed/certified).
 Using safe working procedures and abiding by the 12 Life Saving
Rules and 3 HSE Golden Rules .
 Preparing job hazard analysis for every task.
 Conducting regular job site safety inspections and audits.
 Closing out all outstanding action items from HIR’s and
inspections/audit.
 Providing & enforcing the use of appropriate PPE.
 Providing risk based safety orientation/ training.
 Enforcing safety rules and using appropriate discipline.
 Using a qualified HSE Officer.

Abiding by approved journey management plan. In the event any injury or


emergency, a designated first aider shall be on hand for first line response prior
to being transferred to retainer clinic if necessary.

2.0 Organization and Responsibilities

 Contractor Management:
Although, ultimate responsibility for safety rest with the Managing
Director but the safety requirements stipulated in this plan shall be
implemented and maintained by the Project Engineer supported by
the HSE Officer. The Project Engineer and the HSE Officer are
designated safety critical for jobs to be carried out in the contract.
Their roles and responsibilities and that of other personnel on site are
outlined below.

 Supervisors/Foreman:
 Prepare and sign-off the safety program prior to commencement of
work.
 Organize sites so that the work is carried out as stated in the HSE
Plan.
 Give precise instructions as to the requirements for correct work
method.
 Manage interface with SPDC site rep and subcontractors to avoid any
confusion about areas of responsibility.
 Ensure that fit for purpose equipment are available and in use.
 Periodically patrol the work site to verify that the work is carried out
under safe conditions, with no violations of safety requirements.
 Advice the site Supervisors and Workers of corrective action when
any unsafe conditions or violations are observed.
 Check each work procedure from the safety point of view and advise
the site supervisors before commencement of work and, or while
working.
 Collate, analyze and submit HSE statistics report to site supervisor
and SPDC site rep and make recommendations for safety
improvement (for incident).
 Maintain and share Learning-From-Incidents (LFIs) and other HSE
communications
 Check on the use of all types of personal protective equipment,
evaluate effectiveness and suggest improvements.

5.0 Project HSE Requirements

5.2 Safe Work Permits

5.3 Subcontractor Management: (If Applicable)


Contractors and subcontractors performing work for INDEXPRO shall conduct their
work according to safe and healthful operating practices. They shall follow all applicable
safety and health rules and regulations. Sub-contractor shall be required to develop its
management system in the context of its own organization, but compatible with the
INDEXPRO management system and meeting INDEXPRO minimum requirement.

5.4 Plants, Equipment and Personnel Management

Plants: INDEXPRO has put all plans in place for all her activities in such a way
that her operations shall not jeopardize the existing plants and all jobs shall be
done with care and diligence to ensure that INDEXPRO procedures provided
the system of work is safe with no risk to any existing plants.

Equipment: INDEXPRO shall ensure that her working equipment are suitable
for the purpose of this project, maintained in good order and meet regulatory or
INDEXPRO requirement apart from pre-mobilization inspection and subsequent
follow up. Any known hazard or risk related to deficiencies with work equipment
shall be checked.

Personnel management: The personnel are committed with management


recognition and function that, there can be no meaningful progress without
strong management support. We therefore regard HSE as part of line manager’s
daily responsibilities alongside schedule, financing, good productivity and moral.
And to ensure that all personnel are properly equipped and trained to follow the
safety management system.

Due to the nature of the project, there may be emergency situations, which could require contingency
planning, these include but not limited to;

 Security emergency-a separate security plan has been developed for this project.

In the event of an emergency a distinct site alarm shall be sounded and all workers must promptly move
to the muster point. The type of alarm and position of the muster point shall form part of the site

5.5 Waste Management

The waste shall be segregated at source and moved to site facility waste bin. A Waste

7.0 HSE Communication


HSE Meetings: A monthly site HSE meeting shall be held for all workers and staff. The HSE officer shall
maintain a written record of attendance dated and signed by each employee. INDEXPRO shall produce
the Minutes of Meeting for SPDC within 2 working days.

Toolbox talks: Every morning before start of work a safety Toolbox talk session shall be held by the
HSE Officer and Supervisor with the crew to instruct and discuss:
 Work activity of the day.
 Work procedures.
 Safety instructions for using equipment and tools.
 Particular hazardous / Threatning conditions, controls and recovery measures to be taken for the
work activities.

9.0 Journey Management


The project involves both Land and water transportation. Materials would be moved by road or water
transportation from our base to project worksite while personnel would be moved with crew bus or boat
from base to worksite as well. The following measures shall be put in place to manage transportation
safety.

 All journeys shall be planned and authorized by the Journey Manager


 Any vehicle used shall be pre-mobed and certified fit.
 Any driver shall have valid national driving license and shall have attended the SPDC
DDC/DEP.
 There shall be no night driving (approved hours -0700hrs -1700 hours)
 Where unforeseen circumstances leads to night driving a waiver shall be
obtained.

10.0 Audits, Inspections


Inspections shall be planned for every facility visited, findings, action items, recommendations for
close out shall be done. Internal and external audits shall also be carried out for every facility and
proper documentation shall be made.

11.0 Security plan


Individual security plan shall be develop according to the security situation of the work area and sign-
