This HSE plan covers the incident prevention programs to be employed during
the CONSTRUCTION, REPLACEMENT & REPAIRS OF FLOWLINES IN SPDC
- LAND. All employees shall abide by all the safety rules in this plan including
those that shall be prescribed by Shell Petroleum Development Company
(SPDC).
Contractor Management:
Although, ultimate responsibility for safety rest with the Managing
Director but the safety requirements stipulated in this plan shall be
implemented and maintained by the Project Engineer supported by
the HSE Officer. The Project Engineer and the HSE Officer are
designated safety critical for jobs to be carried out in the contract.
Their roles and responsibilities and that of other personnel on site are
outlined below.
Supervisors/Foreman:
Prepare and sign-off the safety program prior to commencement of
work.
Organize sites so that the work is carried out as stated in the HSE
Plan.
Give precise instructions as to the requirements for correct work
method.
Manage interface with SPDC site rep and subcontractors to avoid any
confusion about areas of responsibility.
Ensure that fit for purpose equipment are available and in use.
Periodically patrol the work site to verify that the work is carried out
under safe conditions, with no violations of safety requirements.
Advice the site Supervisors and Workers of corrective action when
any unsafe conditions or violations are observed.
Check each work procedure from the safety point of view and advise
the site supervisors before commencement of work and, or while
working.
Collate, analyze and submit HSE statistics report to site supervisor
and SPDC site rep and make recommendations for safety
improvement (for incident).
Maintain and share Learning-From-Incidents (LFIs) and other HSE
communications
Check on the use of all types of personal protective equipment,
evaluate effectiveness and suggest improvements.

Site Nurse/Designated First Aider:
Administer first aid to any injured person at site.
Carryout personnel resustation before emergency response is
launched.
Make sure that adequate drugs and first aid box requirements are in
place.

3.0 Scope of work
The key Activities in the execution of this project are but not limited to the
following
Plants: INDEXPRO has put all plans in place for all her activities in such a way
that her operations shall not jeopardize the existing plants and all jobs shall be
done with care and diligence to ensure that INDEXPRO procedures provided
the system of work is safe with no risk to any existing plants.
Equipment: INDEXPRO shall ensure that her working equipment are suitable
for the purpose of this project, maintained in good order and meet regulatory or
INDEXPRO requirement apart from pre-mobilization inspection and subsequent
follow up. Any known hazard or risk related to deficiencies with work equipment
shall be checked.
In the event of an emergency a distinct site alarm shall be sounded and all workers must promptly move
to the muster point. The type of alarm and position of the muster point shall form part of the site
induction. The contact information during emergency and the emergency response chart is attached as
Emergency response plan.

Toolbox talks: Every morning before start of work a safety Toolbox talk session shall be held by the

work activities.
