middle name was erroneously placed therein as “PILAR” instead of her true and

Authenticated Certificate of Live Birth;

4. That the error may have been caused by the inadvertence of the person who

PhilHealth Record, and “JANALYN PILAR RESOR” as erroneously appearing in
the Hospital Record, REFERS TO ONE AND THE SAME PERSON;

6. That we are not related to the said person either by consanguinity or affinity;

7. That we execute this affidavit to attest to the truthfulness of the foregoing

statements and for whatever legal purpose or purpose this may serve under the

JULHUSIN MISUARI BENHAR as erroneously appearing in the PHILHEALTH
Record, and BENHAR MESUARI JULHUSIN as also appearing in the hospital
record, REFERS TO ONE AND THE SAME PERSON;

14. That we are not related to the said person either by consanguinity or affinity;

such that the letter ‘a’ was omitted in his middle name causing it
to appear as PRADERO instead of his true and correct middle
name which is PARADERO and the letter ‘o’ was added to his last
name and what was entered was DELA CRUZO instead of true and
correct surname which is DELA CRUZ.

PERGONE, who died on July 11, 2015 with prior residence at Talon-Talon,
Zamboanga City;

2. That her true and correct full name is MARIA CONCEPCIO SENOGBOJAN
PERGONE, as reflected in her SOCIAL SECURITY SYSTEM (SSS) Record;

3. That however, in her DEATH CERTIFICATE, her name was erroneously placed
therein as “MARIA CONCEPCION SINOGBUHAN PERGONE” inconsistent with
her correct name “MARIA CONCEPCIO SENOGBOJAN PERGONE” reflected in
her SOCIAL SECURITY SYSTEM (SSS) Record;

4. That the error may have been caused by the inadvertence of the person who
prepared her DEATH CERTIFICATE record;

5. That the names “MARIA CONCEPCION SINOGBUHAN PERGONE” appearing in

the SSS Record and “MARIA CONCEPCIO SENOGBOJAN PERGONE” as
erroneously appearing in the Death Certificate, REFERS TO ONE AND THE
SAME PERSON;

6. That we are not related to the said person either by consanguinity or affinity;

7. That we execute this affidavit to attest to the truthfulness of the foregoing

statements and for whatever legal purpose or purpose this may serve under the
law.

We, CARMELITO MARCOS CORONEL and ARNEL SUMALINOG ENRIQUEZ, both of

legal age, Filipinos, and residents of Manicahan, Zamboanga City, Philippines and San Jose,
Gusu, Zamboanga City, Philippines, respectively, after having been sworn, hereby depose
and state:

1. That we know the person of the late RODOLFO S. ENRIQUEZ, who died on
August 14, 2015 with prior residence at Villa Sta. Maria, Zamboanga City;

2. That his true and correct DATE OF BIRTH is “April 12, 1956” as reflected in his
SOCIAL SECURITY SYSTEM (SSS) Record;

3. That however, in his DEATH CERTIFICATE, his birth date was erroneously
placed therein as “APRIL 17, 1956” inconsistent with her correct birth date of
“April 12, 1956” as reflected in his SOCIAL SECURITY SYSTEM (SSS) Record;

4. That the error may have been caused by the inadvertence of the person who
prepared his DEATH CERTIFICATE;

5. That the names RODOLFO S. ENRIQUEZ who was born on “APRIL 17, 1956”
appearing in the Death Certificate and RODOLFO S. ENRIQUEZ who was born on
“April 12, 1956” as appearing in the SSS Record, REFERS TO ONE AND THE
SAME PERSON;
