AFFIDAVIT OF TWO DISINTERESTED PERSON

WE, JAHANNA FAITH B. MINA, of legal age, single and a resident of

Poblacion North, Science City of Muñoz, Nueva Ecija and RACQUEL F.
IBARRA, of legal age, single and a resident of Poblacion South, Science City of
Muñoz, Nueva Ecija under oath, state THAT :

1. We both know personally and intimately know HAZEL JOY L.

ALTAMARINO as we were all working in PhilRice;

2. We have the knowledge that the said items (LG tablet and Canon
Camera) were lost during their travel from El Nido City Terminal to
Puerto Prinsesa City Terminal due to the robbery incident that happened
on October 15, 2017;

3. That after the said incident, HAZEL JOY reported personally to the
Puerto Prinsesa City Police Station, and they conducted requisite
investigation;

4. That we know that HAZEL JOY L. ALTAMARINO do not know who

was/were the person/s who perpetrated the robbery;

5. That we are not in any way related to any of the foregoing person within
the fourth civil degree either by consanguinity or affinity, nor do we have
any pecuniary interest in this matter whatsoever;

6. We executed the foregoing to attest to the truthfulness of the allegations

herein and for whatever legal purpose or purposes this may serve.
