AFFIDAVIT OF SUPPORT AND CONSENT TO TRAVEL

WE, _______________, of legal age, Filipino citizens, and presently residing at

Poblacion South Ramos Tarlac after having been duly sworn to in accordance with
law, depose and state:

1. That we are the biological parents of )_______________ who was born on

January 08, 2007 in Tarlac City.

2. That said child will travel to Australia for the following reason: To attend
the wedding of his aunt;

3. That said child will be leaving on January 02, 2018 and will stay in the said
country for a period of one month and will be taken cared of by his
Grandmother;

4. That we cannot personally travel with my child due to the following

reasons:
That we will be travelling ahead of our child to help in the preparation of my
sister's wedding and the school did not allow our child to travel with us
earlier to avoid long absence from school;

5. That we are giving our full consent for the minor child to travel abroad to
be accompanied by his Grandmother LILIA A. BUENO;

6. That we personally guarantee the financial support of the child in the

course of his travel and stay outside the Philippines and further guarantee
that he will not in any manner become a burden to the Philippine and
Australian governments.

7. That we are executing this affidavit in connection with our child application
for a travel clearance from the Department of Social Welfare and
Development for his travel abroad.
