1. I hereby certify that I am separated and estranged from my spouse

2. I further certify that I do not intend to reconcile with my spouse;

3. Ever since we had separated and up to this date, we have not seen
nor communicated each other;

4. Up to this date, I have no knowledge of her present whereabouts and

as such I do not intend to interfere with her personal life or interfere
any of my personal transactions;

5. I am executing this affidavit to attest to the foregoing facts and for

whatever legal purpose it may serve.
