SUPPLEMENTAL JOINT AFFIDAVIT OF LEGITIMATION

We, EDMAR E. GONZALES and JANETH O. GONZALES, both of legal age,

Filipino, married to each other and residents of Bgy. San Jose, Puerto Princesa City,
under oath, state that:

1. On September 17, 1995, our child MARJANE was born in Puerto Princesa
City and her birth registered at the Civil Registrar’s Office;

2. At the time of her birth, we were not legally married, although we were not
under any legal impediment to marry;

3. We were legally married on April 25, 2001 in the San Jose Baptist Church,
San Jose, Puerto Princesa City;

4. That with mother and father were still minors at the time of conception birth of
MARJANE.

We are executing this affidavit to attest the above facts and to cause the Civil
Registrar of Puerto Princesa City to make the necessary amendments to the Birth
Certificate of our child MARJANE to reflect the fact of her legitimation.
