Dear Engr. Uy,

The attached contract having been approved, notice is hereby given to JMU Construction that work may
commence, effective after the receipt of this notice for the project- Construction and Rehabilitation of
Guibang Barangay Health Center in the amount of Eight Hundred Eighty Four Thousand Five
Hundred Thirty Five Pesos and Eighty Eight Centavos Only (P 884,535.88).

Thus, you shall be responsible in performing the services under the terms and conditions of the contract
and in accordance with the provisions of its bidding documents.

Please acknowledge receipt and acceptance of this notice by signing both copies in the space provided
below. Keep one copy and return the other to the Office of the Municipal Bids and Awards Committee.
