2. That I at my own will decided to Opt-out of the May2020 examination and in lieu of the same
opting to appear in November 2020 examination.

3. That I understand that by making this application I am withdrawing my May2020 examination

application and will NOT appear in the May2020 examination

4. I hereby agree that the details filled in by me in May2020 examination application form will be
carried forward to November2020 examination. In case I do not appear in the November2020
examination I shall not claim any refund for the same.

5. I request that unexpired exemption if any valid for the May2020 examination be carried forward
for my appearance in November2020 examination.
