    I, Muhammad Aman Khan S/o. Haji Muhammad Hassan, resident of Sarwar Shah
    Mohallah, Ward No. 3, Sajawal, to the province of Sindh, holder of ISLAMIC
    REPUBLIC OF PAKISTAN Computerized National Identity Card Number 41408-
    7574112-3, do solemnly declare that:

    1. I am a citizen of ISLAMIC REPUBLIC OF PAKISTAN by Birth and I have not


    obtained the citizenship of any other country.

    2. I am a Pakistan National Person and that I am not a Dual Nationality Person.

    3. Whatever stated above is correct in my knowledge, if I found any wrong


    information which I have submitted I undertake that I shall be held sole
    responsible.
