Your SB providers are ready to provide care through tele-health

using your laptop, tablet, or phone right from home.

Common medical concerns and illnesses can be addressed during a video

visit with your SBHC Provider and/or with a follow-up visit in a Choptank
office only when necessary. Visit reasons could include:
• Medication refills
• Cold and allergy symptoms
• Asthma and breathing problems
• Stomach aches
• Headaches
• Rashes and other skin complaints
• Minor injuries
• Other common complaints.
