WAIVER- TEMPORARY ENROLLMENT

For and in consideration of all the accommodation afforded to me by the College,

I, an applicant for admission to the
program of , do promise to submit the following admission
requirements:

 PSA Authenticated Birth Certificate

 Certificate of Good Moral Character

after the GCQ is lifted or, if and when any Executive Order is issued by the government
to allow physical/face-to-face transactions in educational institutions under the New
Normal scheme.

Further, the conditions specified hereunder shall be applied:

a. That my admission to the College is merely temporary, hence, I am obliged to

submit, on the date specified above, the aforementioned admission requirements
so that I can finally be considered as officially enrolled;

b. That I shall not question, or object to, the recommendation or decision of the
Registrar with regard to my status, nor will I hold the College liable as a result
thereof;

c. That my failure to submit the said requirements will be sufficient cause or reason
for the College to DROP me from the list of enrollees, and that in the event that
my enrollment will be cancelled, I shall not be entitled to any refund;

d. That I hereby affirm that I have read and understood the terms and contents of this
waiver and that I have voluntarily and freely executed the same without any
reservation or objection thereto.
