JOINT AFFIDAVIT OF TWO DISINTERESTED PERSONS

We, CORAZON V. REYES, of legal age, Filipino citizen, and with address at 19-A
Tuna St., Caloocan City, and MARY ANN D. ELTANAL, likewise of legal age, Filipino citizen,
and a resident of Mayor Angeles St. San Roque, San Roque, after having been duly sworn to in
accordance with law, do hereby depose and state that:

1. The we personally know LOUISE ESMERALDA BANDALES ONTOG born on

November 2, 1979 in San Fernando, Masbate to spouses BELLA MANUZON
BANDALES and MARIVELO DIVINA ONTOG both deceased, for having been
neighbors and friends for a period of time;

2. That in her POLICY her birth date was only entered as January 2, 1979 which are
different from November 2, 1979 the true and lawful one;

3. That we hereby certify that the aforesaid birth date is refers to one and the same person
and no other one;

4. That we execute this affidavit to request the proper authorities to correct her birth date
from January 2, 1979 to November 2, 1979;

5. That we execute this affidavit to attest to the truth of the foregoing facts and
circumstances, and to hereby voluntarily and knowingly that the true and correct birth
date which is only a typographical error/misrepresentation, and finally, we executed the
same for all legal intents and purposes.
