CERTIFICATE AND CERTIFICATE OF FITNESS OF TRANSPORT VECHICLE APPLKIICATION FOR DUPLICATE

The certificate of registration (1) and certificate of fitness of my motor vehicle, the registration mark of

I declare that to my knowledge the registration of the vehicle neither the certificate of registration nor
the certificate of fitness has been either suspended or cancelled under any of the provision of Sindh
Motor Vehicle Ordinance, 1965 or the rules made there under:- I herewith deposit the fee mentioned in
the “Fifth Schedule” and apply for the issue of duplicate certificate of registration and certificate of
fitness.
