2. Critically evaluate the measures to improve the outcome of gestational diabetes

mellitus in Sri Lanka.

3. What measures do you recommend to reach the Millennium Development Goal 5

of reducing maternal mortality of Sri Lanka. ?

PAPER II

Answer all six questions.

Each question to be answered in a separate book.

1. A 32 year old gravid 2 in the 32nd week of pregnancy is transferred in a

semiconscious state. She had been dribbling per vagina for 16 days. On
examination her temperature was 380C. She is pale with a pulse rate of 110 and a
BP of 80/50 mm Hg. She demonstrates diffuse abdominal tenderness. What is
your differential diagnosis ? Discuss the immediate management and ongoing
care.

2. A 26-year-old woman is admitted in early labour at 39 weeks gestation, with

severe mitral stenosis and dyspnoea on mild exertion. Discuss the maternal and
fetal implications, and how you would manage the labour and delivery.

3. An 18 yrs old unmarried woman presents to the clinic with a complaint of

hirsutism. Discuss your assessment and management strategy.

4. Evaluate the current minimal access treatment methods for heavy menstrual
bleeding.

5. Give an account on principles of management of post partum eclampsia.

6. Explain how child birth predisposes to post partum voiding difficulties and
develop a strategy to overcome this problem.
