Subject: ANNUAL SUBMISSION OF PHYSICAL CHECK-UP AS

PART OF PERSONNELS’ REQUIREMENT

To ensure efficiency of the delivery of care, all personnel and employees of Peter
Paul Medical Center of Candelaria Inc. shall undergo the annual physical check-up on
any day of April 8 to 12, 2020.
The physical check-up aims to provide a safe environment necessary in
promoting health and wellness while maintaining the quality of care given to the patients
and their family.
Employees are directed to submit the result of the requirement on April 13, 2020
at the office of Human Resource Department.
