All products sent for any Invention, Innovation & Design (IID) competitions are required to register a

Copyright Voluntary Notification with the Intellectual Property Corporation of Malaysia (MyIPO).

WHAT IS A COPYRIGHT VOLUNTARY NOTIFICATION?

A Copyright Voluntary Notification is a prima facie evidence in proving his or her creative works as
owner of his copyrighted works. Copyright Voluntary Notification is an enhancement to the
copyright protection system in Malaysia where a notification of copyrighted work is deposited to
MyIPO. Notification of copyright works can be submitted to MyIPO with a minimal charge (please
refer to the prescribed fee).

EXPECTED OUTPUT
A copy of the completed and processed Notification of Works (Form CR-1) or Notification of
Derivative Works (Form CR-2), and a copy of all supporting documents and receipts must be
submitted to the RIBU UiTM Sarawak personnel upon registration of any IID competition.

WHAT TO PREPARE

1) Form CR-1 OR CR-2.

For Notification of (original) Works, fill in form CR-1.
For Notification of Derivative Works, fill in form CR-2.
The forms can be downloaded form the MyIPO website. CR-1 will be the more common of the two
so it is advised that you check with MyIPO first pertaining to your Type of Work.

2) Statutory Declaration (SD) for Copyright Act 1987 (Act 332)

A template of the SD will be made available. The SD must be completed and signed before the
Magistrate or Commisioner of Oaths. The nearest Magistrate’s Office to UiTM campus is the
Samarahan Resident Office.

3) A copy of your Works, usually tagged as Exhibit ‘A’ in the SD.

(If you have made any Derivative Works, Exhibit ‘A’ will be your Derivative Work, and Exhibit ‘B’ will
be the source of the Original Work.)

4) Payment for Fees.

Please refer to the Fee Table available at MyIPO website. Payment can be made by cash upon
submission at the MyIPO office. Alternatively, payment can be made by cheque / postal order / bank
draft, payable to ‘PERBADANAN HARTA INTELEK MALAYSIA’.

DOES YOUR WORK INVOLVE A TRADE MARK?

If YES, then you will have to fill out two (2) more forms.
If NO, you can skip this step.

For Trade Mark registration, fill out these forms:

1) Application for Registration of a Mark (Form TM5)

2) Statutory Declaration (SD) for Trade Marks Act 1976 (Act 175) and Trade Marks Regulations 1997
A template of the SD will be made available. The SD must be completed and signed before the
Magistrate or Commisioner of Oaths. The nearest Magistrate’s Office to UiTM campus is the
