AFFIDAVIT OF UNDERTAKING

 I, ___ARGIE C. BRIGOLA_______, Filipino, of legal age, married

and with residence and postal address at __Gairan BOGO City
,Cebu_, Cebu, Philippines after having been sworn to, depose and
say:

1) That, I am a _____Teacher 1__ (indicate your position) of ANAPOG

INTEGRATED SCHOOL(indicate your school and/or district), under
DepEd-Division of Cebu Province with Employee ID No.
606-5754.

2) As an employee of DepEd-Division of Cebu Province, I am obligated to

submit my Daily Time Record (DTR) regularly as substantive basis of
my salary;

3) That, however, the whole Province of Cebu is now under Enhanced

Community Quarantine by reason of COVID-19 as mandated by the
Provincial Governor through EO No. 5-N issued on March 25, 2020;

4) That, by reason of the Enhanced Community Quarantine, I will not be

able to submit my DTR on the required period of its submission;

5) That, should there be absences, tardiness or other reasonable basis

for deductions which may be determined upon the actual submission
of my DTRs, I voluntarily consent that said deductions will be taken
from my succeeding salaries;

6) That, I hereby undertake and attest that after the Enhanced

Community Quarantine will be lifted, I will submit my DTRs and all
other necessary documents as needed by the Schools Division Office
of DepEd-Division of Cebu Province;

7) That this Affidavit is executed to attest to the truth of the foregoing

facts and other purposes that this Affidavit may serve.
