SERVICE AGREEMENT

This Agreement is entered into by and between:

PHILIPPINE PORTS AUTHORITY – PORT MANAGEMENT OFFICE OF

BATANGAS, with official address at Port Access Road, Calicanto, Batangas City, Batangas,
represented by JOSELITO O. SINOCRUZ PhD, Port Manager, PMO Batangas, herein
referred to as “FIRST PARTY”,

and

3STAN FASHION HOUSE, a business owned by TRISTAN IRROL R.

HERNANDEZ, with address at Bolbok, Batangas City, Batangas, herein referred to as
“SECOND PARTY”,

WITNESSETH AND AGREED AS FOLLOWS:

1. The First Party is in need of the products and services of the Second Party, to be used by
the PMO Sireyna ng Pantalan representative and chorale members, for the Intra-Cluster
pre-pageant for the SiReyna ng Pantalan and the Chorale Singing Contest on
November 26, 2019;

2. The Second Party has signified his intention, to which the First Party has Accepted, to
provide the following products/services:

a. Sireyna
 Hair and Make Up
 Gown
 Regional/Backdrop Dress
 Accessories
 Evening Gown for Q & A
 Production/Choreography
 Custome for 3 Damas, 1 Igorot, 2 Holders of kawayan

b. Chorale (12 members + 1 conductor)

 Hair and Make Up
 Accessories
 Costume 3 in 1 – Production with Sireyna

3. In consideration of the full, satisfactory and faithful performance of the Second Party of
all its obligations under this Agreement, First Party hereby agrees to pay the Second
Party the amount of Fifty Thousand Pesos (P50,000.00);

4. That the Second Party shall provide dance choreography/lesson/practice to the PMO
Sireyna ng Pantalan representative and back-up dancers from November 19 to 26, 2019;

5. That the Second Party shall perform said service and provide the products on November
26, 2019;
6. That it is understood that this agreement does not create an employer and employee
relationship between the First Party and the Second Party;

IN WITNESS WHEREOF, the parties hereunto signed this Agreement this 18 th day of
November 2019, at Batangas City, Batangas, Philippines.
