JOINT-AFFIDAVIT

I, ALMA A. ROSALES, of legal age, married, Filipino, and resident of,

Brgy. Bato, Biliran, Biliran, after having been duly sworn to in accordance
with law hereby depose and state:

1.) That I am the mother of MARY APRIL A. ROSALES who was born on
April 8, 1989 at FFF, Brgy. Bobongon, Sto. Tomas, Davao del Norte;

2.) That my daughter was born at the said place and not at Brgy. Burabod,
Biliran, Biliran, as reflected in her Secondary Student’s Permanent
Record;

3.) That I know of this fact because I was the one who gave birth to her there
at the said hospital, and that it was only myself and my husband
Sigfredo J. Rosales was present when I gave birth through “hilot”;

4.) That the correct place of birth of my daughter is at FFF, Brgy. Bobongan,
Sto. Tomas, Davao del Norte, as reflected in my daughter’s Certificate of
Live Birth, and not at Brgy. Burabod, Biliran, Biliran, as reflected in her
Secondary Student’s Permanent Record;

5.) That I am executing this affidavit in order to attest to the truthfulness of

all the foregoing averments and for other legal purpose that it may serve.
