    Our supervisor Mr. Ahmed Mohammed Hassan left the work site without informing anyone during work hours. When he left, workers were working without a permit to work (PTW) holder present. To prevent this in the future, the permit to work holder should not leave the site when work is ongoing, and management must take action against any permit holder who leaves work unattended.

    Description of Unsafe Act / Condition :-
    Our supervisor Mr.Ahmed Mohammed Hassan left the site during work time without informing any one.
    During this time our worker were working at site without PTW holder.

    Corrective Action:-

    The PTW holder should not leave the site when the work is going on

    Preventive Action:-

    The management has to take necessary action against the PTW holder in order not to repeat the same.
