Minor injuries, no one was transported for medical attention. I did not issue any citation duc to the river in violation fleeing the scene Vehicle #1 was a white 2003 BMW XS license plate #CRZA628. Prior to my arrival the driver of vehicle #1 had fled the scene. The vehicle was registered to Damien Al snd ene Bsc: I. 0 fn any insurance information inside vehicle #1. Gary's Towing, towed vehicle #l to 945 E Ohio St. ‘Vehicle #2 was a bleck 2016 Jaguar F-Type license plate 4CTX1683/AZ, The driver of vehicle #2 was Xiyan Li Xiyan had a valid Arizona river's license, repistation, and proof of insurance. Xiyan stated he was traveling westbound on Prince in the #1 lane. He was initially stopped at 11d light, the light then cycled to green and he began traveling through the intersection. As he went through the intersection he was struck on the front passenger side by vehicle #1. Xiyan was unsure where vehicle #1 came ftom and did not have a description of the driver. Gary's Towing, towed vebicle #2 away from the scene. ‘Vehicle #3 was a white 2017 GMC Yukon license plate #CDH0333/AZ. The driver of vehicle #3 was James Garliepp TI. James had a valid Asiaona dtiver’s license, registration, and proof of insurance, James stated be was traveling eastbound on Prince inthe I lane, He was intially stopped ata red light, the light cycled to green and he began traveling through the intersection. As he went through the intersection he was struck on the front driver side by vehicle #1. James said vehicle #¥ was traveling southbound on Campbell and first struck vehicle #2 then struc vehicle 13, James described the driver of vehicle #1 as a White male, He had a medium build was 6°, 185 Ibs. He was wearing a white shirt and grey flannel. James did not have any further description. Gary's Towing, towed vehicle #3 away from the scene. | spoke with a witness, Mustafa Nasheed. Mustafa stated he was waiting at the bus stop. He saw the accident but was unable to determine which ‘way the vehicles wore traveling. He sid the trafic signal for cat and west was green at the time of the accident. The driver of vehicle # fled the scene on foot, eastbound on Prince. Mustafa described the driver asa 20-25 yo Hispanic male, He was wearing a white shirt and pants. Mustafa id not have any further description. | spoke with another pair of witnesses, Lance Russell and Heather Mcadaragh, Lance and Heather were in the same vehicle. They were stopped facing eastbound inthe #1 lane behind vebicle #3. The light cycle was initially red but then turned green. Vehicle #3 proceeded through the inersection. Vehicle #2 was traveling westbound and was proceeding through the intersection as well. Gamer (49543) took photographs. | took futher photogranks of vehicle #1 and submitted them into the case. I also observed what appeared tobe possible blood on the outside door handle of vehicle #1. {swabbed the outside door handle, steering wheel, and gear shifter for possible suspect DNA. I submited the possible suspect DNA swabs into evidence, [ submitted vehicle #1 into evidence. I also contacted TWX, TWX Operator #97730 entered the vehicles location into the system, ‘iyan and James both wanted to prosecute. The officers used force against Alvarado in order to detain him, up t and including the use of a TASER, TFD units were called tothe scene to assess Alvarado aftr the use oforee, and they determined that he did not need to be transported toa hospital. Alvarado had been tased and a spit sock had been placed over his head during his arest. TFD was called and responded to this location where they administered medical tests, Alvarado was not transported and as TFD was leaving the seene, officer noticed Alvarado bad stopped responding, CPR was administered and TFD returned to provide medical aid. Alvarado was transported to UMC, however was pronounced deceased at 1830 rs Sergeant Kerlin of Gun Crimes asiste in this investigation as did his personnel, Homicide Detectives Robinson and Alskog also responded and took lead 00 this ease, ISDU had already been called to collect BWC, Upon arriving atthe scene, approximately 2030 hrs, all focus and witness officers were still prosent and had boon provided cover officers, [Liewtenant Brady was IC. Once the focus officers (Solarino, Ake, Yeadie, Gomez, and Gracillss) were identitied, round counts were conducted and their BWCs were collected. The citizens were ing to old ooto the suspect when officers from Midtown anived and took over attempting to rake the suspect ino custody, The suspect conte to fight with the Midtown officers and Ore. Gamee from ODW ran over tothe seene to assist the Midtown officers ‘Once they had the suspect handeusfed, TED responcled to make sure that hs injures did oot require medical care. The suspect continued to fight with the TED personnel that were trying to check him over for injusies and determined his physical condition, Set, Ellis advised me thatthe TFD ‘unit told them thatthe arrestee was fine for transport co the jail and they started to leave. Just as they were leaving Sgt. Elis looked over and saw ‘that the suspect had stopped breathing and the officers had started CPR. Guticrrez and advised him that I needed nine detectives to act as cover officers and to have them respond to Olsen south of Prince, While responding to the scene I contacted Ls James Brady to determi if'a Midkoven commander was responding and he advised me tht he was responding. Upon arrival I met with Captain Dennison, who had taken over incident command. 1 advised Sgt. Evans that | had arranged for NCS etoctives to respond and take over as cover officers forthe officers that required it.I instrcted him to hove the officers that were relieved as cover officers ta cheek the area for any potential surveillance video, It was determined that thee were four, possibly six focus officers and the remtining officers were only witness officers. The crime scene unit was already taking photographs of the officers that needed to be photographed. Case Number: 2003220124 Date: 3/23/2020 18:50:03 Supplement Notes BWC used in accordance with GOs 1 was on an unzelated ‘on N Oracle and heard this incident on the radio and acknowledged ic with dispatch, Officers responded and I heard ‘Sei. Evans mention that officers on 93 were fighting with someone, 278 responded code 3 and I acknowledged that. I was attempting to finish ‘he call on Oracle and heard Sgt, Evans request more units as he monitored 93, I started ceiving that direction and switched may vehicle radio to 93, but was unable to determine what was happening, 1 approached the intersection of Prince/Camphell and sew three vehicles in the interseetion. Officers were dealing the collision. I pieced together that one ofthe divers from the collision had fled on foot and citizens followed him to an area near Olsen/Prince and officers were fighting with the suspect driver, Radio broadcast that dhey were near the church at 1990 E Prince, I went ‘there and saw Officer Jackson and Officer Vance behind the church. I checked with them and they were OK and talking tothe two citizens that Followed the male driver that fled on Fox. I then looked south and saw more police vehicles down the alley way, Lalked tht way and sa that officers had detained the male on the ground, The male was sil resisting eventhough he was handcuffed othe rear and a tarp was on his ankles, | verbally checked on the officers, They out of breath and ther clothing was dirty. Three of the officers were from ODM and Officer Gamer from ODW was there, One of the ODM officers had ealed for TFD to check the male. One ofthe ODM officers mentioned that be had deployed bis Taser onthe male, The officer mentioned that an ODM sergeant was going to respand, ‘TED arrived and the male was stil stugeling against officers attempts to keep him on the ground. I directed them to apply a spit sock on the male. The male resisted that and cusediyelled I moved aside t allow TED personnel to treat the male. Sgt. Evans was onscene by that point. 1 spoke fo one of the TFD personnel with the computer and he told me that they were not going to transport the male. Officers involved inthe fight with the male began taking photos of their uniforms and injuries. Officer Durazo and Officer Vance were holding the male down, TED began to clear and walked towards their vehicles, We noticed the male was not moving and I directed them to check on itn. An officer felt for a pulse on his neck and didnot find one. We directed the officers to roll him onto his back and begin chest compressions. We called for TFD to return. They had net left the alle area, yet ‘They retumed and officers removed the trp and handeuftS. The spit had been removed by that point. Officers continued chest compressions until paramedics arrived.
