AFFIDAVIT OF LOSS

I, RACHEL GIANGAN MAYOL, of legal age, Filipino, married and resident of Blk. 2,
Lacap Subd., Prk. Malakas, General Santos City, after having been duly sworn to in accordance
with law, do hereby depose and say:

1. That I have in my possession for safekeeping the Company Identification Card (ID)
of my sister Sharon Giangan issued by Mindanao Medical Center, General Santos
City;
2. That my sister is already working in Abu Dhabi;
3. That she told me to send to her the said Identification Card (ID), which was
required by her employer in Abu Dhabi;
4. That when I looked over for the said ID, I found out that it was no longer in my files
where I kept it;
5. That diligent efforts exerted to locate said Identification Card (ID) but it proved
futile as the same could no longer be found or located;
6. That I am executing this affidavit to attest to the truth of the foregoing facts and for
whatever purpose this may legally serve.

Avenue, Dad. Heights, General Santos City, after having been duly sworn to in accordance with
law, do hereby depose and say:

1. That I am a holder of Plane Ticket route: Manila-Kuala Lumpur-Manila, which I

purchased through Marazul Travel, Inc.-Marisa Bautista;
2. That the just recently, when I looked over for said plane ticket, the same could not be
found;
3. That diligent efforts were exerted to locate my said plane ticket, proved futile as the same
could no longer be found or located;
4. That I am executing this affidavit to attest to the truth of the foregoing facts and for
whatever purpose this may legally serve.
