 Incident Details
On the day of May 6, 2016 at around 10:00 AM at the Site 1 – Recycling
Plant, the 5HP Low Pressure Pump Motor for Hot Press temperature was
reportedly became hot while running. Together with Engr. Irwin Balanquit, we
immediately went to the plant to check the motor that is being complained. We
conducted a test and we check all the parameters except the internal part of the
motor. The following are the data we gathered:

 Motor Specifications as per nameplate:

Brand - New BHARAT (India)
Power in Watts - 3.7 kW
Power in HP - 5 HP
Voltage - 440 V
Full Load Current (FLA) - 7.8 A
Frequency - 60 Hz
Efficiency - 84%
Connection - Delta
 Motor Outer physical check ( May 6, 2019, 10:15 AM)
- Cable termination is not loose
- There is no short circuit between the wires
- The Motor mounting is intact

 1. Motor Run test at Full Load for 30 seconds ( May 6, 2019, 10:20 AM )

- Motor temperature is above than normal after running.

- There is no suspicious noise while running.

- There is no suspicious noise while running.

 3. Motor Run test at No Load for 10 minutes ( May 7, 2019, 3:10 PM – 3:20 PM )

- Motor temperature is above than normal after running.

- There is no suspicious noise while running.

Recommendation
 Always put extra caution working on height. Secure equipment’s and its
loose parts to avoid accidental fall.
