Fall through the grating opening

□ Summary and Causes of Incident

When scaffolder
prepared installation of temporary pipe
support on the pipe rack, he lost his
balance and fell to the ground (7m)
through the grating opening (1m*0.7m)

SECL construction & safety team
* S-PCM was performed, but the
safety measures for grating
removal were not reviewed well
by construction SI.
- The training for grating opening
and S-PCM were not conducted.
* Omitted safety work process
- It takes time to discuss
interface work, due to the
work restriction of
subcontractor (Covid-19)
* There is no proper
communication Channel for
brown field work activities
- Use the client’s work permit.
※ There is no Grating removal
permit from client
- There is no review process for
the client’s permit by SECL SI

Finger caught-in between Girder and Web-Splice

□ Overview and Cause

▷ Date : 2020. 06. 05(Fri) 15:58

▷ Description
While worker was trying to hold the position of web splice on the bolting point, the second
worker was holding web splice which is attached to a rope. The worker who was positioning
web splice for bolting was holding web splice with hands, resulting in pinching finger in
between girder and web splice due to poor communication among holding and positioning
workers(Web Splice : 0.4*1.4M, 3T, 75Kg)

□ Preventive Measures
- Thoroughly tighten the web splice to the girders from the ground so that there is no play / space
between the girders and the web splice.
- The bolt position shall be kept inwards to achieve further tightening if there is play
- Adjustment of Web Splice shall be carried out by using attached ropes NOT directly by hands.
- Worker holding the web splice and adjusting girder both shall have better communication
- Wear the right type of gloves to prevent hand injuries.
