Innovations at IIT Bombay for COVID-19

Dear Alum,
 
In response to the COVID – 19 global pandemic emergencies, which has
generated an outpouring of concern and financial support from IIT Bom-
bay alumni community, the Institute has created several options for peo-
ple to help address this public health crisis. It includes providing equip-
ment, space, expertise and other resources to the health care facilities. It
will also support the work of manufacturing the PPE (Personal Protective
Equipment). Contributions also help in other aspects of fighting the pan-
demic like vaccine development, portable ventilators, AI solutions, and
improved protective equipment. Attached is a list of funding opportuni-
ties for your reference.
 
We would greatly appreciate if you could contribute for this cause and
help us go the extra mile towards a safer world.

Washable/ Reusable PPE Kits: These are TPU infusion coated polyester which
are highly breathable suited for medical doctors and health professionals and are
washable up to 30 cycles (under the guidance and suggestions of INMAS DRDO).
We are looking into an additional option of making the PPE antiviral in nature
using Nano material dispersion.

Porous mucous membranes in your mouth, nose, and eyes are the entry points
for pathogens like coronavirus, flu and the common cold. You should take care
every time you touch your face. There is always a risk of infection if you touch your
face without having hand washed.
Unconsciously, a person touches his face 23 times per hour on average. Because
of the same reason, a huge population in India including many doctors might be
getting affected by the coronavirus even after taking maximum precautions.
Faceguard is a device which needs to be worn on the hands and calibrated with
personalized algorithms using some simple steps. It will start producing vibration
as soon as the person is going to touch his face to make him/her cautious. This will
prevent the unconscious touch and the probability of getting infected.

• We developed a digital stethoscope- AyuSynk, which allows wireless listening


of heart and lung sounds - thus avoiding contact between patient and doctor. It
helps in early identification and monitoring of Pulmonary and Cardiovascular
diseases characterized by Abnormal lung sounds and heart sounds.
• In COVID condition it is difficult to use conventional stethoscopes to avoid
patient to doctor infection transmission. Further, COVID-19 patients having
symptoms of pneumonia have characteristic lung sounds called Crackle and
wheezes. Also, patients with underlying heart and lung diseases have the
highest mortality. Stethoscopes are a simple tool for doctors to perform
physical examination. AyuSynk helps in monitoring of the heart and lung
sounds even keeping safe distance from the patients by using the Bluetooth
capability of the device. It can also record, visualize and analyse breathing
sounds, making it useful for screening, diagnosis and monitoring the patients.
• The company has supplied 1000 units so far, most of them to Physicians,
rural PHCs and some of them recently to KEM and other hospitals dealing
with COVID patients.
• Now the company needs funds to scale up production and support to reach
out more doctors and hospitals.
• Also we have deployed more than 10 Smart OPDs in hospitals in Mumbai.

In current scenario, many innovators are working on making a low-cost ventilator,


due to shortage of resource, innovators have limitation in choosing a design.
Since we are stationed in the automotive hub of Pune, we thought of going
with a model that can be made with locally available standard components.
We are using a pulley based design with a guide, so that we can get a
reliable model which can also be scaled for manufacturing with locally available
aluminium and linear guides.
The most critical part is to have a reliable design, which should have the basic
functionalities of the ventilator, such as having a controlled breathing profile
and should be able to increase the O2 level gradually whenever needed. The
unique abilities of the design are translated into the following points:
• It can give primary breathing support in terms of scarcity of oxygen or no
oxygen available in stage three scenarios when oxygen cylinders would be
luxury for the patients.

• It can give a battery backup of 4 to 5 hours to the patients and has provision to
add an external car/bike/truck battery if required in case of emergency.
• It enables the doctor to adjust the Tidal Volume and Breathe Per Minute by
observing the display.
• Careful profiling of the inhalation and exhalation can be done using variable
control measures, which can be suited to any required combination as per
patients’ condition.
• Dual-use H.M.E. filter to avoid the infection from the corona patient to the
doctors or other patients.
• Face shields as an additional accessory for the medical attendant and doctors
to avoid the infection during treatment.

Isolation room for bed ridden patients, this unit can be set up on the bed
itself enclosing the patient. The unit has provision for ventilation, personnel and
material access with containing infection. This can convert any hospital bed into
a BSL-3/BSL-4(BSL-Bio Safety Level) isolation unit. This will be an ideal solution
to isolate and treat patients with infectious diseases. Efficient air-filtration and
climate control module makes sure the air entering and leaving the module is
contamination free with patient comfort. The unit will be fitted with a surgical
gown like structures for doctors’/ caretakers entry and this unit will be mounted
on to each bed enclosing the patient. Special boxes are attached on either side of
the enclosure for instrument /material transfer. These features ensure effective
treatment without any change of cross-infections.

We have developed an AI algorithm that can automatically detect COVID-19


abnormalities from chest X-Rays and CT scans. The developed AI algorithm
is validated at Tata Memorial Hospital. Our AI can facilitate the screening of
potential COVID-19 patients and the monitoring of confirmed COVID-19 cases.
It will impact patient flow, wait times, resource utilization and allocation, and
risk-stratification.

Targeted next generation sequencing for precise and high-throughput


detection of SARS-CoV-2. This project aims to detect COVID-19 infection using
NGS (Next Generation Sequencing) based approach. This method will reduce
the effects of false positives/false negatives on the sequencing results and
provide better understanding of the disease pathobiology, mutation rate
and coinfections. NGS for COVID 19 detection will improve accuracy and also
sequencing of numerous samples can be done by pooling techniques. Such a
study has not been reported anywhere in the country till date.

Its portable Hand free hand wash device. Device is IR sensor based and get
activated by just waving hand on top of device. At first dilute soap liquid flow
for 15 sec and then water flows for 10 sec (Time is tuneable). Hence it make
sure user wash hand for 25 sec without coming in contact with device. It does
not require any water line connection can be placed in bucket or water tank.

Remote, At-Scale, Detection-Intervention-Prevention of Covid-19 transmission.


Current Covid-19 epidemiology, using community healthcare workers for
undertaking extended interrogation of Covid-19 patients and extensive on-foot
contact, significantly increases the risk of disease for health workers, and can
also make them as carriers of the disease. Here we propose to implement a
contact-less “Remote, At-Scale, Detection-Intervention-Prevention of Covid-19
transmission”, based on Genomics and Geospatial mapping.

• The product uses germicidal properties of UVC light (253.7 nm) and ozone
(O3) gas to disinfect/sterilize equipment and closed spaces. It infuses O3 into
a closed airflow in the first step post which it is directed into an enclosed
UVC chamber. This chamber can contain equipment for sterilization or
disinfection. The disinfected air is released into the room post the second
step. The disinfection time is automatically controlled. The working
mechanism is explained below.
• Ozone destroys viruses by diffusing through the protein coat into the nucleic
acid core, damaging the viral RNA & destroying the capsid or exterior protein
shell through oxidation (protein hydroxides and hydro peroxides). Ozone
interferes with bacterial metabolism & destroys them by inhibiting operation
of their enzymatic control system and breaking through their cell membrane.

In the second step, UVC radiation warps the structure of viral genetic material
and prevents viruses from making more copies of themselves. Bacteria and
microbial DNA are disrupted by destroying nucleic acids leaving them unable
to perform vital cellular functions.
We don’t rely on unsafe O3 gas dispersal into a room (the control over
concentration and air mixture homogeneity of which is difficult as well as critical
for human safety and effective disinfection). Our closed loop provides effective
control & breaks down excess O3 into safe oxygen in the UV step before release.
Our enclosed UVC step avoids human exposure.

• During the COVID-19 pandemic, there is a compelling need to identify clinical


predictors of severe COVID-19 to enable risk stratification and optimize
resource allocation. It is essential to make judicious decisions regarding the
usage of available infrastructure, especially ventilators.
MetFlux proposes Respiratory Performance Prediction Platform (RP3), an
innovative Systems Physiology-based modular digital solution, to assess
respiratory health in COVID-19 patients.
• It can assess respiratory function dynamically and can predict the probability
of improvement, relapse and/or failure of lungs,
• Time series data linked from the ventilator will improve model
performance at the individual level to give clinical assistance to doctors.
It will assist health professionals to make efficient clinical decisions and
thereby provide real-time ventilator requirements at the hospital level to
utilize the available resources optimally.

In view of Covid19 outbreak its’ imperative to maintain personal hygiene in
places where substantial amount of public gathering takes place like offices,
societies, shops, malls etc.
By introducing sanitization of hands (which is highest source of contamination)
at the entry gate itself, we can prevent further spread of infection. We propose
automatic sanitizer dispensing machine with following features:
• Sensor based hand detection
• Alarm indicator once requisite quantity is dispensed
• Sanitizer spray in mist form which reduced quantity necessary for complete
hand wetting
• 3D spray allowing all sides of hand sanitized (and not only palms)
• Portable and light weight
• Microprocessor based circuit makes program easily portable according to liquid
properties and quantity needed to be dispensed
Future proposal- Can be linked to attendance system via App or suitable means and
make it compulsory to sanitize hands before entering office or factory premises.
Advantages:
• Contact free dispensing- no need to touch the equipment for sanitizer
dispenser
• Mist spray - Spray in the form of mist thus reducing wastage of liquid, only
half ml of sanitizer is sufficient for wetting of both the hands
• Microprocessor based circuit- Additional features can be incorporated for
better control

The idea is to use photocatalytic active nanomaterials against COVID19.


Photocatalytic active Nanomaterials have a property of generating free
radicals of OH in presence of sunlight and atmospheric water vapour. These OH
radical are responsible for the antimicrobial action of nanoparticle. These OH
radical breaks the protein structure of virus and hence make them inactive.
We have proven antimicrobial activity against Bacteria testing and reporting
done by Bombay textile Research Association. Many researchers around the
world have as mentioned in reference section have used our nanomaterial for
killing of virus including SARS virus. The vision is to bring photocatalytic active
Nanomaterial for fight against current pandemic of COVID19.
Innovative Steps
• We have developed a special process for incorporating of the nanomaterials on
fabric surface. We have scaled the technology and have the patent in pending
stage. This makes the fabric antiviral and antibacterial as reported by Bombay
textile Research Association. We have produced the fabric on industrial scale
using propriety chemicals and at factory in bulk quantity and hence we can
produce the fabric in as much quantity needed provided we are given support.
This fabric will protect the patent and the doctor from COVID virus as it is
expected to inactive all virus in 30 min of exposure.
• We have developed a special process specifically for COVID scenario of
disinfectant Here we form stable Nano dispersion of nanomaterials and imbibe
them on surface. This feature helps in making a surface disinfectant against
virus and bacteria for months as the nanomaterials keep on generating the OH
radicals using atmospheric water and sunlight and hence killing the virus by
rupturing it protein structure. The patent is applied for the technology and it is
developed in-house in Navork Innovations Lab at SINE IIT Bombay.

The Key feature of the products are
• The nanomaterial used in product TIo2 is reported to break the protein
structure of COVID19.
• Product production process is already scaled when it comes to bringing it on
fabric surface or when used in disinfectant
• It keeps the surface free from COVID19 for at least a month as a sunlight
exposure of 30 min is needed to kill all viruses.
• Process can be easily integrated in existing Industry
It uses easily available chemicals in industry to bring it on fabric surface or in
disinfectant.

The product is the hand-held portable electronically operated disinfectant


sprayer for surface disinfection. A single fluid nozzle is used to generate aerated
mist to dispense the disinfectant. The product is developed to disinfect indoor
spaces like office and home, as well as objects like chairs, tables, files and money
which are in contact with humans. The sprayer automizes the solution into the
micron-sized droplets, this increases the effectiveness of disinfectant to spread
evenly into the air and react with the micro-organisms and neutralize the surface.
It sanitises the surface with minimum wastage.

Safe and pleasant hand wash. While this is an infinitely huge market, there
are equally huge number of sanitizer manufacturers and there is a bloodbath
on quality and rates at which the sanitizers are being traded. Piscium has tied
up with Xpressbee and home delivers sanitizers across India. It is also in the
process of tie ups with amazon, pinkblue.com and medikabazaar. The new age
logistic channels are much more cost efficient and deliver higher realisations.
Currently Piscium needs funding of about 20-25 lacs for marketing spends
and working capital.

20 QAWaCh Bio Pvt Ltd

QAWaCh Bio proposes the development of two diagnostic tests for detection
of COVID-19 infection. First is called InstaCOVID-19, CoV-2 screening test
and the second is InstaCOVID-19, IgG/IgM confirmatory test. InstaCOVID-19,
CoV-2 screening test detects viral load in nasal swab or saliva sample and is
reliable for early screening of COVID-19 infection, is non-invasive in nature,
with a turn-around time of only 15 minutes and would cost around INR 150-
200. Our second proposed test is InstaCOVID-19; IgG/IgM confirmatory test
is again a point of care test that uses serum to detect the titre of antibodies
against Corona virus in the sample. InstaCOVID-19, IgG/IgM confirmatory test,
in comparison to RT-PCR, does not require any sophisticated lab infrastructure
for its working and gives results within 15 minutes. This test would cost around
INR 200-300 per sample.

We have been developing graphene based materials which can be used


to make cartridge type full face respirators or masks. Larger size air purifiers
have been demonstrated using this. We have tested the material for removal
of PM2.5, PM10 and various gases such as NH3, SO2, CO2 etc. where we could
use the material by back flushing with inert gasses. The material is also capable
of removing odour. While same material during water purification is able to
neutralize bacteria, No studies have been performed to see the effectiveness
against any airborne biological agents such as bacteria, viruses etc.

A telemedicine solution along with a mobile app that will manage the possible
spread of COVID-19 pandemic.
• MOBILE APP: A self-assessment questionnaire of symptoms and signs of
coronavirus to study the prevalence of the infection in the community. This will
help the Government to plan containment strategies and prepare necessary
resources to tackle any emergent situation on a war footing.
• TELEMEDICINE: Through Telemedicine solution, patients will be evaluated for
symptoms at primary care, write any medically necessary prescriptions and
arrange for suspect patients to be tested at an authorized facility closest to
them Benefits of the solution: 1) The solution will avoid overcrowding and rush
of patients to Tertiary Hospital and cities this avoiding unnecessary travel and
out of pocket expenses - Maintain social distancing 2) This will free necessary
manpower resources (Healthcare Workers) at Tertiary Hospitals so they can
attend to the duties of looking after sicker patients.

The solution is a cloud-based mobility healthcare platform with features of


electronic medical records, interactive audio-video sessions with clinical notes in
standard SOAP format, e-prescription, patient-monitoring, dashboard & triaging,
and collaboration among the doctors, paramedics, phlebotomists and pharmacists.
The solution is HIPAA compliant. It embeds the care protocol for Covid.
It allows a district level organization to create partitions for the zonal teams to
implement the care protocols. It supports the Covid-19 stages, the workflows and
corresponding care protocols. It supports a native mobile client for the patient.
The patient’s vitals and subjective health indicators are regularly captured in the
app. It supports collaborative messaging and co-ordination among the dispersed
clinical team members with a mobile client.
• When deployed in a district, this will enable the district nodal health officer
to medically care for and monitor tens of thousands of people who may
be asymptomatic (suspected or confirmed) cases and mild cases in their
homes or dispersed quarantine centres. When shifted to isolation wards
under moderate symptoms, the same team can extend their care using the
platform. This will enable use of hospital beds only for those with severe
symptoms and help augment the capacity of healthcare significantly during
the course of the epidemic.

Cleaning/Municipality waste pickers are at much greater risk. There is high risk in
quarantine centers and industries/offices which will lack such incineration system
and cleaning staff not trained in handling such contaminated medical waste.
• There is an urgent need for the development of portable and small scale
incinerator which can be used in such places.
• Portable incinerator is a combustion device. It is a forced draft combustion
device with main objective to ensure complete and clean combustion and
ensuring minimizing of soot (PM), UHC and CO emissions.

37 Short-term projection of COVID-19 medical
resources and inventory

• The medical inventory projection web application originated from


discussions with a district magistrate to get a projection of the resource
gap as the disease progressed.
• The underlying model uses common worldwide trends seen in cases and
mortality to make predictions at the district level.
• Detailed medical inventory was planned by medical professionals in the
Armed Forces Medical Services. 
• Combining this with the model, we are able to give approximate numbers
of staff, equipment and consumables, for up to four weeks from the latest
available confirmed data.
• The application uses the raw data from covid19india.org and provides the
predictions computed on the fly on the users’ web browser without any
backend database or server-end computations.
• Web application developed to provide a four-week projected requirement
for various medical inventory across districts, states and at national level
