Login Page
STEP 3: Upon successful login, you will be prompted with SLU Data Privacy Policy. Take time to
read and click Download Consent Form before you proceed with the Enrollment. The SLU Data Privacy
Consent Form shall be sent along with your required documents stated in STEP 6.
After downloading the Consent Form, go back to Enrollment Site Tab and proceed with the
Enrollment.
STEP 4.2. Complete your Student Information. Your information will be used for reporting
purposes of the School and to communicate with you easily especially during these times.
Complete all the fields to proceed with the next step. Enter N/A or dash (-) if not applicable.
STEP 4.2.1: Click “Submit and Proceed” at the end of the page after entering all your
information.
STEP 4.3. After submitting, you will be redirected to the first page of Enrollment. Select
your details then Click Save and Assess again to proceed with the Assessment of Fees.
STEP 4.4: On the next page, scroll down to click “Compute and Print” button to load
Assessment of Fees.
STEP 4.4.1: You can choose to print or save your assessment for your copy.

STEP 4.5: Please click the link below Compute and Print button. The link should redirect you to
a google form for DepEd Survey. Kindly take the survey as it will be reflected in the DepEd’s
board.
STEP 4.6: Go back to the Portal page after taking the Survey then click Exit to close the
Enrollment page.
After completing these steps, you may now logout from your student portal and proceed
with the payment.
STEP 5: Proceed with Payment (pay the required down payment to confirm your enrollment)

Print and Fill Out the form, scan and send it to rlpalabrica@slu.edu.ph
