2. Become familiar with the behavioral criteria described in each of the levels.

3. Self assess your performance continually, relative to the Professional Behaviors, using the behavioral criteria.

4. At midterm and at the end of the internship, complete this form.


a) Using a Highlighter Pen, highlight all criteria that describes behaviors you demonstrate in Beginning (column 1), Intermediate
(column 2), Entry Level (column 3) or Post-Entry Level Professional Behaviors.
b) Identify the level within which you predominately function.
c) Document specific examples of when you demonstrated behaviors from the highest level highlighted. Please use different color
pens to denote midterm vs. final.
d) For each Professional Behavior, list the areas in which you wish to improve. Please use different color pens to denote midterm
vs. final.

5. Share your self assessment with your clinical instructor, specifically seeking his/her feedback.

**Professional Behaviors were developed by Warren May, Laurie Kontney and Annette Iglarsh (2010) as an update to the Generic Abilities.

1. Critical Thinking - The ability to question logically; identify, generate and evaluate elements of logical argument; recognize and differentiate facts, appropriate
or faulty inferences, and assumptions; and distinguish relevant from irrelevant information. The ability to appropriately utilize, analyze, and critically evaluate
scientific evidence to develop a logical argument, and to identify and determine the impact of bias on the decision making process.

Examples of behaviors to support my self assessment:

Critical analyses was one of the things I struggled with in my outpatient setting, however I was able to learn and make accurate analysis and hypotheses. I applied
my current knowledge to situations and learned from mistakes. I acknowledge the presence of contraindications with every patient when necessary and used
evidence based research with my treatments.

Regarding this Professional Behavior, I would like to improve in the following ways:

I would like to continue to expand my knowledge and with that become more confident in my decision making and thought rationale.

Examples of behaviors to support my self assessment:

Communication is one of my strengths between other disciplines as well as with my patients. I welcome constructive feedback, because my goal as a
therapist is to constantly be improving and learning. I enjoy working with others and collaborating and have been able to improve on group work throughout
my educational experience. I have room to improve in being persuasive with my communication which ties to some of my lack of confidence.

Regarding this Professional Behavior, I would like to improve in the following ways:

I would like to improve on my communication through difficult situations which comes with practice and experience.

Examples of behaviors to support my self assessment:

During problematic situations I like to consider the possible consequences of the solutions and look to others for guidance and feedback. I accept
responsibility for any decisions on my part for solutions and I look to evaluate the outcomes and make sure that was the correct solution. However, with my
lack of experience, I tend to look to others in certain situations for guidance and resources to solve problems.

Regarding this Professional Behavior, I would like to improve in the following ways:

I would like to improve in being able to modify solutions to improve on the outcomes and be fully independent in problem solving.

One of my strengths is listening and communicating with others as well as building rapport with coworkers and patients. I have learned how to pick up
on non-verbal cues and help patients when they may not know how to ask for help.

Regarding this Professional Behavior, I would like to improve in the following ways:
I look forward to establishing a mentor relationship in entry and post entry level. I also would like to improve on my conflict management and resolution
skills which can be a weakness of mine depending on the situation

Examples of behaviors to support my self assessment:

I take full responsibility for all my actions and encourage patients to take control of their situation and be accountable to themselves. Education is
another one of my strengths and proving to patients why it is important for them to know how to perform things correctly and safely. I have relied on
my clinical instructors for some clinical decision.

Regarding this Professional Behavior, I would like to improve in the following ways:

I would like to become a leader in my workplace and be accountable for all my clinical decision. I would also like to become a clinical instructor and
have the responsibility of educating future therapists.

Time efficiency has been a struggle of mine with documentation in both settings, however have been able to make improvements by the end of the
clinical rotations. In group educational work I have been able to discuss with the group in strategies to be the most effective and productive

I have had the opportunity to learn from evidence based research articles and have found them to be good resources in problem solving. I still have
trouble understanding some research and applying it to treatment/intervention.
