I. Statement of Company Policy

Ashton Sawing & Drilling (ASD) is dedicated to the protection of its employees from on-the-
job injuries. All employees of ASD have the responsibility to work safely on the job. The
purpose of this plan is: (a) To supplement our standard safety policy by providing safety
standards specifically designed to cover fall protection on this job and; (b) to ensure that
each employee is trained and made aware of the safety provisions which are to be
implemented by this plan prior to the start of construction.

This Fall Protection Plan addresses the use of conventional fall protection devices and there
use at a number of areas on the project as well as identifying specific activities and the
safety measures that will be taken while work is performed. These areas include:

-Connecting activity
-Rebar Installation @ Walls
-Form Installation & Removal @ Walls
-Leading Edge @ Upper Levels
-Confined Space Access
-Rescue

II. Implementation of Fall Protection Plan

The structure being erected is a multistory cast-in-place concrete structure consisting of a


pit, mat/base slab, vertical walls and horizontal upper levels.

The following is a list of the situational hazards and measures that will be taken to overcome
them:
Walls

Walls will be constructed of vertical and horizontal rebar closed in by panel forms. For
Rebar installation the employee will use a certified fall protection harness with 4’ double
lanyard (or SRL) and a rebar positioning device. The employee will maintain 100% tie off
using the above mentioned items secured at all times to the rebar. Once the forms have
been installed to a point that the rebar is not useable the above operations will continue
except tie off will be to the formwork. (This process will be the same for the 1 st & 2nd level
walls). Walk platforms will be constructed as part of the form system once able and work
will be done from these as able. While on the walk platform (with OSHA compliant mid rail
and top rail) the workers will be allowed to move without tie off. (Work platforms will be
inspected prior to use by ASD competent person and inspected daily prior to use. Access to
the platforms will be by extension ladder.

Horizontal Upper Levels

Where able a warning line system will be used. A 6’ perimeter will be established and
marked in accordance with OSHA and work inside this area will not require fall protection. A
safe path shall be established for access in and out of the 6’ perimeter area in accordance
with OSHA. Work outside the 6’ perimeter will be done using leading edge certified SRL’s
attached to approved anchor points.

III. Pit Access

Access to the interior area will be made using extension ladders or OSHA approved “Job
Built Ladders”. The ladders will be secured accordingly.

Work in the pit area will be done under the supervision of a hole watch. The hole watch will
remain outside the pit area at a vantage point where he can monitor the work being
performed. The hole watch will have a 2-way radio and an air horn. In the event a rescue is
needed he will communicate via the 2 way radio and 3 long blasts of the air horn. This will
signal for all work to stop and he will then notify site personnel and/or 911 as deemed
necessary.

IV. Rescue

Rescue from the lower pit area will be provided by the crane. While work is being
performed in the base of the pit the crane hook will remain available for rescue operations.
A rescue basket will remain in or as close as possible to the lower area of the pit at all times
work is being performed there.
