OVERSEAS FILIPINO WORKER (ROF)

STEP 2: PROCEED TO CITY/MUNICIPAL HEALTH OFFICE; (FOR MEDICAL HEALTH

CERTIFICATE)
a. That the LSI/ROFs is neither a contact suspect or probable or confirmed COVID Cases;

STEP 3: Applicants must submit/attach the following;

* LETTER REQUEST FROM APPLICANT/REQUESTING PARTY INDICATING THE FOLLOWING:
a. Names with Contact Number:
b. Point of Origin and Destination;
c. Travel Date;
d. Purpose;
e. Details of Vehicle with Plate number (O.R / CR) and Photocopy of Driver’s License of the
Driver.

STEP 4:
a. SUMBIT ALL THE DOCUMENTS TO THIS OFFICE AT S3-POMU RIZAL POLICE PROVINCIAL
OFFICE, CAMP MGEN LICERIO GERONIMO BRGY. DOLORES TAYTAY RIZAL.
