EDC Terminal Rental Purchased

I / We hereby give you one (1) month notice to terminate our Merchant Agreement with Paydee Sdn Bhd. The reason for termination
is due to: ____________________________________________________________________________________

Kindly arrange to collect the rented EDC terminal (s) at this address;

I / we understand that pursuant to my EDC Terminal Rental Agreement, I will pay the penalty fees as stipulated in the Terminal Rental
Agreement / Sales Contract, if the terminal is not returned to Paydee Sdn Bhd or the authorized vendor.

I / We confirm that the Clauses relating to Chargeback by Paydee Sdn Bhd as stated in the Merchant Agreement will continue to
apply. In the event Paydee Sdn Bhd effects a chargeback pursuant to these Clauses, I / We shall pay upon such demand to Paydee
Sdn Bhd the full sum including any fees at the rate imposed or to be imposed from the time to time by Mastercard Worldwide, Visa
International or Credit Card Company, as the case maybe.

I / We hereby agree that notwithstanding the aforesaid, Paydee Sdn Bhd rights and entitlement under the Merchant Agreement,
including its rights and entitlement of Chargeback hereof against the Merchant shall survive the termination of this Agreement for
or relating to any Payment Card transaction or Transaction Slip presented by the Merchant prior to the date of termination.
