    1. Request approval of PAFCPIC Salary Loan in favor of PFC Jersie F


    Servicio 946461 (Inf) PA of this unit.
    2. The subject EP has a capital contribution of five thousand (₱ 5,000.00)
    pesos in PAFCPIC.
    3. Subject EP will utilize the proceeds of the said loan on PAFCPIC to
    purchase agricultural land at Sitio Sangkugi, Poblacion, Veruela, Agusan del Sur.
    4. Furthermore, subject EP will have a net take home pay of nineteen
    thousand six hundred eighty two and fifty five (₱ 19,682.55) after all deductions has
    been made.
    5. Base on unit’s record, the subject EP has no pending case as of this
    date.
    6. Request favorable consideration.
