Apply for Exam:

Computerized Examination
The Pharmacy exam is a dual application process.

If you wish to be licensed in Illinois by examination and you have not taken and successfully passed NAPLEX
and the MPJE OR if you have applied for a NAPLEX Score Transfer from NABP within 90 days from sitting for
the NAPLEX examination in a different state, please complete the online application. Please note that your
Score Transfer cannot be more than 1 year old.

Please keep in mind that as of January 16, 2007 a dual application process is required. This means you must
apply to this office by completing the online application, pay the required application fee(s) and then apply
and register with the National Association Boards of Pharmacy (NABP) and pay your exam fee(s) at
www.nabp.net. Please make sure to do this simultaneously in order to receive your Authorization to Test
(ATT). FAILURE TO REGISTER WITH NABP AT THE SAME TIME AS YOU REGSITER WITH THIS OFFICE
WILL RESULT IN A DELAY OF YOUR RECIEVEING AN ATT.

Although, if you are currently licensed in another state and you have not applied for a score transfer to Illinois
within 90 days of taking your NAPLEX or your score transfer is more than 1 year old. DO NOT apply to this
office. Please follow the Licensed by Endorsement instructions listed below.

APPLICATION FOR PHARMACIST LICENSE BY ENDORSEMENT:

Illinois uses the National Association of Boards of Pharmacy Preliminary Application for Transfer of
Pharmaceutical Licensure for the purpose of issuing a license on the basis of endorsement. This form can be
obtained directly from the National Association of Boards of Pharmacy (NABP) at www.nabp.net or by
contacting the Department of Financial and Professional Regulation:

SUPPORTING DOCUMENT: ED-PHM FORM

The education form must be completed by the Dean of your College or University and must include the date of
graduation or expected date of graduation. This form can only be submitted in the last semester prior to
graduation (not more than 4 months prior to the date of your graduation). Please note, if you choose to pre
file your education form EDPHM please be advised that once you pass both the NAPLEX and MPJE
examinations you will be required to submit actual proof of graduation to the Illinois Department of Financial
and Professional Regulation prior to the issuance of your state license. This does mean your school will need
to submit a new completed EDPHM form to the Illinois Department of Financial and Professional Regulation
showing proof of your actual graduation. If they’re any other documents required by this office prior to
approval you will be notified.
SUPPORTING DOCUMENTS REQUIRED TO BE SUBMITTED PRIOR TO APPROVAL OF EXAMINATION:

 A Certification of Education Form ED-PHM is required. This form is available at:

APPROVAL:

Once you have completed both processes and are determined eligible by this office to sit for examination
NABP will be notified and Pearson VUE will issue an Authorization to Test (ATT) within 5 business days. This
ATT will include your authorization to test number and the information required to schedule your
examination(s).

Note:
If you applied online and paid with a credit card then it’s not necessary for you to submit a copy of your
application to this office. Although, if you completed your application online and chose to submit your
payment by mail then it is mandatory that you print the application off, sign and date it and then mail to this
office with your payment and supporting document(s).

**Application fees paid to this office are non-refundable.
