SWORN STATEMENT WITH AUTHORITY TO DEDUCT

I, MARIA KRIZIA NICOLE R. GREGORIO, Filipino, single, of

legal age, a resident of 20 Guyabano Street, Brgy Quirino 2-B, Project 2
Quezon City, and an employee of LizardBear Tasking, Inc. (“Company”) a
domestic corporation with principal place of business at Twenty-Four Seven
McKinley Building, 24th Street corner 7th Avenue, Bonifacio Global City,
Taguig 1634, hereby depose and state that:

1. I am an applicant of TaskUs NextGen Scholarship Program

(hereinafter referred to as “Scholarship Program”) and that said
application is made and entered into solely for the purpose of seeking
educational assistance in behalf of my children, KRISTOFF
DURDEN CALYX G. PEREZ, and KEI VIKTOR EROS G.
PEREZ (hereinafter referred to as the “Beneficiaries”);

2. All details and documents I submitted during the application process

(“Application Documents”) of the Scholarship Program and those
submitted after the approval thereof, including documents required by
the Company for liquidation purposes, (“Post-Approval Documents”)
are true and correct based on records and my personal knowledge;

3. I am hereby authorizing the Company and any of its authorized agents

and/or representatives to verify and/or validate the authenticity and
veracity of the Application Documents and Post-Approval
Documents;

4. The scholarship amount approved (“Scholarship Amount”) shall only

be used for the purpose of paying the tuition fees and other fees
related to the education of the Beneficiaries. I understand that the
misuse or misappropriation of the Scholarship Amount has criminal
and civil implications under the laws of the Republic of the
 Philippines and that the Company may pursue me for the commission
thereof; and

5. The Company may, at its discretion, allow the deduction of the

Scholarship Amount from my salary/payroll in equal monthly
installments until 31 December 2020. I understand that the staggered
deduction of said amount from my salary is a privilege that
management may withdraw upon just cause.

6. However, if I have failed to submit Post-Approval Documents or if

the same is found to be either tampered, forged, or counterfeited, I am
giving the Company or any of its authorized agents or representatives,
the authority to deduct from my salary/payroll account the
Scholarship Amount in full, subject to reasonable notice, or run after
me, whether criminally or civilly, for the remaining balance or
unliquidated amount, the Scholarship Amount.
