LINE OF ORGANIZATION CHART AND LAYOUT OF

HOSPITAL WITH COMPARISON TO ACTUAL HOSPITAL

Definition Of Organization:

An organizational structure consists of activities such as task allocation, coordination and


supervision, which are directed towards the achievement of organizational aims.

According to Mintzberg ,”Organisational structure is defined as the sum total of the ways in
which its labour is divided into distinct tasks and then coordination is achieved among these
tasks.

Developing The Organizational Structures:

Three types of organizational structures exist:

a) BUREAUCRATIC
b) MATRIX
c) FLAT

Nursing organizations often combine characteristics from the three types, forming a structure
that is a hybrid. Shared governance is a term often used to describe the flat types of
structures presently being designed to meet the changing needs of nursing organizations.

1. BUREAUCRACY: Bureaucracy evolved from early theories on organizing work. It


arose at the time of societal development when services were in short supply, worker’s
and client’s knowledge bases were limited and technologies for sharing information were
undeveloped. Characteristics of Bureaucracy arose out of a need to control workers and
were centered around division of processes into discrete tasks. These are formal,
centralized and hierarchical and consists of division of labour and specialist .Rules,
Standards and protocols ensures uniform actions and limit individualization of services
and variance in workers performance. Communication and decisions flow from top to
bottom, limiting employes autonomy.

At the time Bureaucracies were developed, these characteristics promoted efficiency and
production. As the knowledge base of the population and employes grew and
technologies developed, this structure is no longer fit the evolving situation. Increasingly ,
employees and consumers functioning in Bureaucratic situations complain of red tape,
procedural delays and general frustration.

An organization can demonstrate Bureaucratic characteristics in some areas and not in


others. A method in determining the extent to which Bureaucratic tendencies exist in
organizations is to assess the organizational characteristics of labour specialization
,centralization(at what level of the organization decisions regarding carrying out work and
remuneration for work are made)and formalization(what percentage of actions are
required to deliver patient care is governed by written policy and procedures

Bureaucratic procedures are commonly called LINE STRUCTURES. Line Structures


usually have a staff component. Line Structure have vertical line, designating reporting
and decision making responsibility, that connects all positions to a centralized authority.

Line Functions are those that involve direct responsibility for accomplishing the
objectives of a nursing department, service or unit. Staff Functions are those that assists
the line in accomplishing the objectives of a primary objectives. Line Positions may
include RN, liscenced practical/vocational nurses, and unliscensed assistive personnel
who have the responsibility for carrying out all aspects of direct care .Staff positions may
include staff development personnels, researchers, and special clinical consultants who
are responsible for supporting line positions through activities of consultation, education,
role modeling and knowledge development, with no authority of decision making. Line
personnel have authority for decision making, whereas staff provides support ,advice and
counsel. To make the line and staff functions effective, the authority for decision making
is clearly spelled out in positions description.

2. Matrix Structures:

These are designed to focus on both product and function. FUNCTION is defined as all the
tasks required to produce a product that is designated as the end result of the function. In a
matrix organization, the manager of a unit is responsible for a service reports both to a
functional manager and to a product manager. For eg.a director of pediatric nursing could
report both to a chief executive officer(Product manager)and to a vice president of
nursing(functional manager)
Matrix structures have been effective in the current health care environment. It is a hybrid
structure that combines both a bureaucratic structure and a flat structure. It superimposes a
horizontal program management over the traditional vertical hierarchy. Personnels from
various departments are assigned to a specific program and become responsible to two bosses-
their functional department head and a program manager. This creates an interdisciplinary
team.
A line manager and a project manager functions collaboratively. Nursing care is delievered in
a teamwork setting or within a collaborative model. The nurse is responsible to a nurse
manager for nursing care and or to a program/project manager when working in the matrix
overlay. Well developed collaboration and coordination skills are essential to effective
functioning in a matrix structure. One example of the matrix structure is the patient focused
care delivery model that is being implemented in some facilities.
IHBAS-Institute Of Human Behaviour And Allied Sciences,Delhi

Introduction:

Institute of Human Behaviour & Allied Sciences (IHBAS) was established in compliance
with the directives of the Hon'ble Supreme Court in response to a public interest litigation in
1993. The Institute is an autonomous body registered under the Societies Act 1860, funded
jointly by Ministry of Health and Family Welfare, Government of India and Government of
NCT of Delhi. As an autonomous body, the institute has its Memorandum of Association
and Rules and Regulations duly approved under the Societies Act. Minister for Health, Govt.
of NCT of Delhi is the President and Chief Secretary, Govt. of NCT of Delhi is the
Chairman of the Executive Council.

Vision:

The vision of Institute of Human Behaviour and Allied Sciences is to be one of the leaders in
the country and in the world for promoting mental health, neurosciences, behavioural and
allied sciences through multidisciplinary patient care in partnership with communities, 
developing future group of professionals and paraprofessional leaders through formal and
semiformal training programs and expanding scientific knowledge and its application through
frontline research.
Mission:

The mission of Institute of Human Behavior and Allied Sciences is to advance the mental and
neurological health of populations by practicing evidence based multidisciplinary scientific
health care approaches that are novel, adaptive, and ethical, conduct research of high standard
and develop human resource in these fields. The institute adheres to quality assurance,
integrating into practice the prevailing norms for human rights and diverse value systems of
persons and populations while accomplishing this mission.
To accomplish its vision and mission, IHBAS will do the following :
  -  Form a strategic Review and planning Group (SRPG) which will include external group
members (expert and   people of repute from the field of administration, finance, research and
academia) assisted by IHBAS personnel. 
  - Ensure that the institute amalgate and synthesises a comprehensive vision document for
the institute based on the vision of all the departments/sections of the institute.
  - Work with various govt, nongovernmental and voluntary health organizations and
other interested stakeholders to reduce the discrimenation and prejudices associated with
mental illnesses and neurological disorders.
  - Promote the visibility and mandate of the institute and its contribution through effective
communication with all sectors of civil society.
HBAS is committed to provide Health Care par excellence. This would be achieved
through :

2. Working within the framework of Regulatory requirements.
3. Upgradation of Technology and Facilities on regular basis.
4. Training the manpower in the relevant discipline.
5. Continuous Quality improvement.
6. In the process, striving to enhanced both staff and user satisfaction level.

The Aims & Objectives of the institute are:

1. To provide high quality super specialty health care using state-of-art technology in
Mental health, Neurosciences, Behavioral and Allied sciences.
2. To conduct research of high standard in the field of Mental health, Neurosciences and
Behavioral sciences.
3. To provide comprehensive training for MD, DM, M. Phil. & Ph.D courses in field of
Mental health, Neurosciences and Behavioral sciences and research activities on
current interest.

Various cost effective measures have been taken to improve services. Quality of patient care
has improved remarkably. Duration of stay of patients has been reduced. Good quality
essential drugs are provided to the patients free of cost by the institute.

PHYSICAL LAYOUT OF NIMHANS COMPLEX

Introduction:
The National Institute of Mental Health and Neurosciences is a multidisciplinary
institute for patient care and academic pursuit in the frontier area of mental health and
neuroscience. The Lunatic Asylum which came into being in the latter part of the 19 th
century was renamed as Mental Hospital in 1925 by the Ministry of Government,
Mysore. This hospital and All India Institute of Mental Health established in 1954 by
Government of India and was amalgamated on 27 th December, 1974 and this was formed
as the autonomous NIMHANS. The priority gradient adapted at the institute is service,
manpower development and research. On November 14 th1994 NIMHANS has been
declared as Deemed University by the University Grants Commission, with academic
autonomy. The Institute functions under the direction of ministry of health and family
welfare Government of India and ministry of health and family welfare, Government of
Karnataka. Several national and international funding organizations provide resources
for research.

Objectives Of The Clinical Posting:


 To apply the nursing process in the care of patients with mental disorders in hospital
and community.
 To demonstrate advanced skills/ competence in nursing management of patients with
mental disorders.
 To identify and care for special groups like children, adolescents, women, elderly,
abused and neglected, people living with HIV/AIDS.
 To identify and manage psychiatric emergencies.
 To provide nursing care to critically ill patients with mental disorders.
 To utilize the recent technology and various treatment modalities in the management
of patients with mental disorders.
 Demonstrate skills in carrying out crisis interventions.
 To identify areas of research in the field of psychiatric nursing.
 To prepare a design for layouts and describe standards for management of psychiatry
units/emergency units/ hospitals.
Mission:

o Evolve strategies to offer diagnostic and therapeutic facilities to all corners of


India in the field of Mental Health and Neurosciences utilizing the advances in
information technology and to be the nodal center.

o Establish training facilities and impart knowledge in the field of Mental health and
Neurosciences to all the developing countries by Institutional and Distance
Learning.

o Establish state-of-the-art diagnostic and therapeutic facilities in neurological


disorders with special reference to genetically transmitted disorders and
vaccinology of tropical infectious diseases and be the nodal center for the Country
in Research and Development.

o Develop strategies for Disaster Management and Psychological Rehabilitation.

o Integrate physical and metaphysical aspects of Neuroscience Research.

o Participate in Neuroscience and Behavioral Research related to Space, Atomic


energy and Transplantation.

o Be part of world leadership in the field of Neuroscience and Behavioral science.

Location Of NIMHANS:

NIMHANS is situated in Hosur road which is about 8 km from the Bangalore city
railway station and 11km from the Bangalore airport. The hospital is spread over an area.
The hospital is well laid down with plenty of gardens for patients and their relatives.
