The patient reported difficulty breathing and was exhibiting signs like using accessory muscles, nasal flaring, and wheezing. Their vital signs were taken and they had an elevated respiratory rate of 30 bpm. To treat this, the patient was encouraged to drink fluids, do deep breathing exercises, receive chest physiotherapy and repositioning, and was given IV fluids and medications as prescribed to relieve their difficulty breathing and allow them to sleep for 8 hours.

Encouraged Deep Breathing Exercise Chest physiotherapy 30 minutes before meal Reposition the patient to semi/high fowler’s position every 2 hours PNSS 600 cc regulated @ 22-23 gtts. Meds given as prescribed by the physician.
R: The patient is able to breath normally ranges 16 to 20 bpm
The patient relieved and able to sleep with a minimum of 8 hours Continue monitoring.
