Check that formwork installation had been proper,
4 release agent applied and meets the requirement as
per the approved design drawing.
Check that reinforcement has been erected as per

drawing/specification
Check that all civil piping has been fixed as per

drawing/specification
Check that all electrical items/embeds are fixed properly

as per drawing/specification
Check that all embedded items, box outs, metal
8 sleeves, bolts and dowel bars have been fixed in
position as per drawing/specification
Check that provisions for grout/formwork has been
9
done
Check that area has been cleaned and proper access

been provided
Check if more than one source of steel is used. If yes
11
provide details

All setting out verified by Surveyor including concreting Surveyor Name & Sign
