Safe Work Procedure
4. Definition : Nil
5. Responsibility :
5.1 The Person In-Charge (PIC) / Project Manager / Co odinator / Site Engineer are
overall responsible for all the activities of the bored piling activities, machineries
and lifting equipments used at worksite.
5.2 Lifting Supervisor (LS) is responsible for the supervision or lifting operation
control of all lifting activities.
5.3 The Safety Personnel (wshc or SS) is responsible for regular monitoring of the
legal requirements.
5.4 The crane, boring and excavator operators are responsible for maintenance of the
daily checklist.
6. Procedure :
Before excavation works, existing ground installations like sewers, gas pipes, water
pipes and electrical cables shall be detected by location and depth so that excavation
works will not damage these services. Avoid causing any damage of these
installations by taking precautions applicable to the particular line. For instance
where electrical cables could be damaged by excavators, the machine should not be
used where it reaches the depth of a cable. This would not only save cost in repairs,
but also prevent a case of electrocution of the worker besides cause inconvenience to
the public and industries.
Site Engineer shall submit the crane access design calculation with Professional
Engineer endorsement. Site Engineer shall submit weekly crane access checklist to
main-con safety personnel.

Site Engineer shall submit the design calculation with Professional Engineer for
temporary casing lifting point based on the length and size of casing to be used for
piling work.
Boring rig / crane shall be assembled / dismantled with the supervision of Approved
Crane Erector and examined by Approved Examiner and has valid LM certificate.
Only operator with valid license / certificate allowed operating the boring rig.
Operator shall lock the cabin and hold the key with him whenever leaves the
machine to prevent unauthorized person operating the machine.
Boring rig has parked on hard ground with steel plate to prevent toppling /
overturning of boring rig during work and travelling.
Proper maintenance of boring rig to minimize the noise generated from worn parts
of machine and environmental pollution due to the emission of dark smoke. Obtain
monthly maintenance certificate / service report from mechanic / servicing
company.
Boring shall be provided with danger sign board and blinker light to alert the
workers.
Provide adequate swing / work space for the boring machine to swing in order to
prevent collision with other machinery / equipment / workman.
No person shall be allowed to stay near the boring rig during work and pile
confirmation should be done after complete halt of boring rig as the flying earth /
soil may injure the workman.
Crane piling equipment must be operated by competent and qualified operator and
supervised by lifting supervisor and safety officer.
Height of piling equipment lower than viaducts are allowed to operate under the
viaducts and in 1st reserve.
PE or PE assignee to conduct daily inspection the MRT zone area and standing
supervision of boring work within 1st & 2nd reserve line.
Joining of steel casing shall be done by qualified welder to avoid breaking of casing
during driving / extracting from borehole using vibro hammer and use of appropriate
type of vibro hammer based on the length and size of casing required for the work.

Steel casing must be protruding at least 1.0m above ground to prevent person falling
into bore hole. Concreted bored hole should be backfilled and well compacted to
prevent person falling in and injured by protruding steel bar.
Banks man shall be provided with luminous vest, whistle and safety goggles.
Operator shall check the Kelly bar wire continuously during drilling for wear and
tear. If any defect found in the wire, work should be stopped immediately and
replace with new wire rope to prevent dropping of Kelly bar into borehole and also
workman may injure by burst wire rope.
Pre-bored hole shall not be left uncovered / barricaded to prevent person falling into.
Conduct tool box meeting to instruct and educate banks man on safe practice of
changing the boring tools to avoid finger caught in between boring tools and Kelly
bar.
Steel cage lifting point shall be designed by Engineer and welded accordingly by
qualified welder. Suitable lifting gear shall used to hoist and install the steel cage
into borehole.
Concrete truck access should be provided with steel plate to ensure lorry wheels not
to stick in soft ground spot.
Banks man shall signal the lorry driver while reversing and ensure no person
standing direct behind the truck.
Entire site team should be briefed on Safe work procedure of boring rig operation
and Risk Assessment of bored piling work by their supervisor.
Boring rig / crane / excavator shall be equipped with fire extinguisher. Banksman
should always watch for fire hazard during the course of work.
Ensure that the electrical equipment have LEW sticker which check by competence
person.
Must have good communication each other when lift a piece of rebar.
For banding machine must competence and trained person can allowed to use.
For cutting machine must trained person able to operate the machine.
Have in house training for all activities at last the worker know a major scope what
they do
All the workers must follow the SWL(not more than 20kg/person)during the manual
lifting of straight bar or inserting the straight bar into the bar bending machine.
To stop bar bending machine immediately if the straight bar stuck inside the
machine and the fabrication only be resumed after the abovementioned condition are
clear.
To make sure the working platform for the fabrication of of rebar cage are satisfied
suitable for works.
To clear all the combustible materials away from the hot work area if using the gas
cut for rebar.
To direct the movement of heavy vehicle during the hoisting of rebar cage upon the
completion.
Lifting Machine
Crane shall be marked with its safe working load (maximum and minimum radius),
and the LM number or other means of identification.
Crane shall be examined by an Authorized Examiner after any major repair or any
repair done on the hoisting or operating mechanism, boom/jib and after over loading
incident.
No crane shall be operated unless all measures are taken to prevent toppling or
overturning or contact with overhead obstruction (such as structure, power line)
When the crane is moving, the boom shall be lowered and an authorized
person/signalman/bank man shall be required to guide the crane operator.
When crane is reversing, an additional person shall be required to ensure that the
path of travel is clear.
Crane cabin shall be locked when it is not attended by the operator. Unauthorized
person shall not be permitted to enter the cabin or operate the crane.
Fire extinguisher shall be provided in the crane. Operator shall be trained to use it.
Safe access (with proper foot and hand holds/supports) to the cabin and deck of the
crane shall be provided and identified.
Crane shall be maintained to ensure that all components of the lifting and operating
mechanism are in good, safe and sound functioning condition.
The operator shall carry out daily inspection of the crane and testing of the safety
devices in accordance to safety's requirements before the operation of the crane
begins.
The maintenance section of respective supplier shall inspect crane at least once in
every 3 month in accordance to safety's requirements.
Lifting Operation
Lifting plan and procedure (or according to Company Safe Work Practice) for
review prior to the execution of the lifting operation.
Establish lifting plan shall be briefed to all lifting crew before lifting work comma
cent on site.
The lifting supervisor must verify that the strength of lifting gears and equipment are
adequate and their certified SWL are in line with the load they bear.
The lifting operation area and other areas with potential hazard / danger must be
cordon off to prevent access for all unauthorized personnel. Only authorized persons
and personnel involved in the lift are allowed in the lifting operation area.
Safety measures shall be taken to address any potential hazards and/or danger that
may arise out of the lifting operation.
The operator shall be able to observe the load continually during lifting.
The load shall be controlled by means of tag lines to prevent the load from turning
or swinging.
Hoisting or lowering loose materials such as stones, bricks, slates, bolts and nuts,
shall be done in covered containers or receptacles, and to be secured firmly.

Competent, trained and authorized persons shall perform lifting and crane operation,
banking, slinging and signaling.
Only the appointed signalman is allowed to give signals and signs to the crane
operator.
All lifting gear must be identifiable and checked before and after use.
Lifting beam or spreader-bar shall be used for hoisting long materials, to space the
sling legs for proper balance.
When a load is suspended at two or more points with slings, the eyes of the lifting
legs or the slings shall be shackled together and placed on the hook or shackled
directly to the hoisting block, ball or balance beam, or placed on the lifting hook
without shackles it the hook is provided with safety features for such use.
Crane Operator
Crane operator shall have full knowledge of the lifting plan and/or procedure any
lifting operation.
Crane operator shall be responsible for the lifting job carried out by him, and as such
he shall:
have full knowledge of the total weight of the load (including lifting gear or
content of any hollow) the crane will be bearing during a lift.
check and ensure that the load is correctly and securely rigged / sling to bear
the load by authorized and competent person under the supervision of a lifting
supervisor.
ensure a tag-line(s) is to be attached and. secured to the load. It is used to guide
the load.
have full view of the load and the path taken by load continually at all times.
check clearance of the path to be taken by the load and any part of the crane.
test lift the load before carrying out the lifting job;
take signal commands form the assigned / appointed signalman only, and shall
comply with the signals, unless otherwise he believes that the signal given will
have adverse effects on the safety of the lift/ crane, or he foresees any accident
potentials. He must stop the operation and call the lifting supervisor to review
the signal given;

comply and carry out the safe work practices for lifting machine;
ensure safe and sound working condition of the crane, and to operate the crane
in a manner (as he had been trained) that does not endanger life or property;
Crane operator shall inspect the lifting machine and gears for safe operation every
day prior to commencement of work.
Crane operator, by himself, can decide to halt the lifting job, and / or in consultation
with his supervisor is empowered to abort any lifting job or operation, with regard to
any change of situation or condition which have adverse effects on safe working
load, the safety of the lift / crane, or if he foresees accident potentials.
All lifting gears to be checked for damages, wear and tear, missing components
like the safety latch from the hook etc. by the rigger and signalman and counter
checked by Lifting Supervisor before use.
Lifting gears should be inspected and certified by approved person once in every
6 months. If the lifting gear is used for man cage then it should inspected and
certified once in every 6 months.
The colour code system to be followed closely as per the in house requirements
by the rigger and signalman. Lifting Supervisor to ensure the compliance.
If there is damage or more than 5% of the wire sling strains, the lifting gear must
be immediately discarded.
The tag should be provided by the supplier / main contractor / the sub contractor
identifying the LG Number, SWL, Expiry date, Company details of the lifting
gear.
Padding must be provided if the lifting gear is seated on the sharp objects.

Rigger to select correct type and capacity of lifting gear for the load to be lifted
and Lifting Supervisor to ensure the above.
Monitoring
The designated Person In-Charge (PIC) and supervisors shall inspect and ensure
proper usage of the lifting gears at the workplace / worksite regularly.
Ensure that the electrical equipment have LEW sticker which check by competence
person.
Must have good communication each other when lift a piece of rebar.
For banding machine must competence and trained person can allowed to use.
For cutting machine must trained person able to operate the machine.

To direct the movement of heavy vehicle during the hoisting of rebar cage upon the
completion.
Before setting up of kentledge base, existing ground installations like sewers, gas
pipes, water pipes and electrical cables shall be detected by location and depth so
that the weight of kentledge will not damage these services. Any damage incurred to
existing ground services may not only endanger the safety of workers in the pit but
also cause interruption of services to the neighboring houses, schools and
commercial buildings.
Supervisor shall ensure the availability of a valid permit prior to allowing such
works. Lifting operation area shall be cordoned off and warning sign board placed at
conspicuous location to prevent unauthorized entry.
Site Engineer shall analyse the ground condition and prepare the design calculation
endorsed by Professional Engineer to ensure the ground condition to carry the load
effectively without endangering safety and health of workers.
The design calculation shall indicate the base hardcore thickness, number of steel
plate, main beam and secondary beams etc.,
Supervisor shall ensure the Kentledge base is free from soft soil and mud before
start to lay hardcore and well compaction of hardcore.
Supervisor shall ensure the kentledge base is prepared according to the Professional
Engineer design.
Crane operator with valid MOM registered license. Qualified lifting supervisor,
Rigger and Signalman shall be engaged to do lifting operation. Provide training /
briefing to lifting team on the procedure of kentledge set up.
Appropriate lifting gear with valid approved person certified shall be used.
Crane operator and lifting supervisor shall ensure the crane is parked on even and
hard ground with steel plates placed perpendicular to the track.
Crane operator and lifting supervisor shall ensure the crane’s mechanical parts and
safety devices are in working condition.

Lifting supervisor, crane operator and rigger shall be known the weight of load to be
lifted and ensure the crane or lifting gear shall not be overloaded in any case.
Rigger and signalman shall be attire with red helmet and vest.
Tagline shall be used to control the load on lengthier materials like steel beams and
platforms.
Man cage with valid Approved person certificate shall be used to ensure safe means
of access and egress for the workers working at height.
Workers doing work at height shall be provided with safety harness and lifeline to
secure it.
Rigger shall check and ensure the test block steel lifting hook is not damaged or
worn out seriously before start to hoist. If any defect found in lifting hook shall be
informed to lifting supervisor immediately.
Crane operator vision shall not be obstructed during the stacking / removal of test
block and Lifting supervisor shall ensure the communication between the operator
and signalman.
Test blocks shall be stacked in such a way that it is fully sit on the secondary beams
and shall not be extended out of secondary beams at any time.
Stage 1 - Safe Work Procedure For Test Block Stacking And Unstacking

Test blocks are place at both sides of the steel frames to facilitate the workers
to arrange the secondary girders.
After all the secondary girders have been properly arranged, two live-lines are
attached to the ends of the secondary girders. The safety harness of the worker
is hooked onto one of the live-lines.
During lifting of the individual block the rigger will position himself at a
corner away from the placement of the test blocks with safety harness secured.
Tag lines will be used to guide the test blocks into position.

6.3 Stage 2 - Safe Work Procedure For Test Block Stacking And Unstacking
When arranging of the test blocks, the live-line are hooked onto the tests blocks
as shown in the above.
For the second to the last levels of test blocks, this process of detaching the
live-lines and re-attaching to the next higher of tests blocks is repeated.
When the last level of test blocks is arranged, the life-line is removed and a
‘Man Cage’ will be used. (refer to attached SWP for the use of man-cage).
Unloading of test blocks will start from centre in a ring towards the perimeter with
whole sequence exactly the reverse of the stacking procedure.
Lifting supervisor to inspect all lifting gear prior to slinging on the lifting materials
and hooks.
All personnel must be cleared from the top of the load prior to lifting.
No personnel must be under the suspended load or within the vicinity during
hoisting.
Provide proper access to personnel to claim up trailer /lorry and co worker to hold
the ladder .
all personnel using safety boot/safety shoe make sure clean up during claim up
trailer/lorry.
Make sure all personnel wearing personnel protective equipment at all time,(Eg,
safety boot/safety shoe , helmet, chins trap hand gloves, safety vest)
Ensure that the LE tag is engrave or hook on to the man-cage for identification
purpose.
Workers inside man-cage must hook on the safety harness to the man-cage
before cage is being lift to the designated area.
Ensure that other lifting appliances such as shackles and sling are all test by AP
before allowed to be used.
PDA TEST:
Ensure PDA testing area must be clear from others obstruction by any
materials/structure.
Lifting supervisor rigger signal man & safety personnel to be in present during PDA
test.
Conduct daily checks to ensure proper fixing of mole point at the tip of mechanical
breaker.
Conduct daily checks on the conditions of breaker/ air compressor hose and
connections.
Wear appropriate ppe ( eg safety helmet, safety boot , & goggles, hand gloves .
Excavator operators shall possess valid excavator operator certificate, pub license
and safety orientation certificate (SOC).
Excavator operator shall conduct daily check and submit the daily excavator
operator checklist to safety personnel before commencing work.
Excavator operator shall ensure the excavator is installed with right and left view
mirror at all times.

Excavator operator shall ensure no paper, cardboard, etc, shall be pasted or placed
on the window screen of excavator to ensure all angle visibility.
No excavator shall be used for hoisting or dragging of load unless designed and
approved by a professional engineer (P.E.) and issued LM certificate.
Banks man to proper controlling excavator operating movement during pile hack by
excavator breaker.
Supervisors shall assign a signalman / worker who shall stand at a safe distance in
front of the excavator’s extended arm to direct the movement of the excavator or to
warn workers to keep their distance from the moving excavator.
When the excavator is in operation, all workers shall maintain a safe distance away
from the excavator and during cutting the pile.
When excavator is use for hacking work, excavator operator, worker and signalman
must wear ear plugs and breathing mask.
All supervisors are required to brief their respective workers pertaining to the
hazards of working close to excavator.
Ensure pile cutting machine is good condition before start cutting pile.
For installation of crack inducer with rebar cage , if any, the workers must use
proper ppe.(eg hand gloves ,safety goggles )
