    Dear Fellow Teachers,

    As per DepEd Memorandum No. 80, s. 2015, month of January is the schedule of 2 nd batch of
    deworming of students which is a yearly activity. In view of this, 4 copies (per section) of the
    Parents’ consent will be distributed for the transferred-in students that were not given a copy of the
    consent last July.

    We are expecting the Deworming Activity in our school anytime next week. Please prepare
    and/or secure/retrieve the parents’ consent of your respective classes before the deworming
    schedule. Please remember that the Department of Education aims a 100% dewormed students.

    Thank you for your usual cooperation.
