AFFIDAVIT FOR CHANGE OF BODY OF

MOTOR VEHICLE
I, GIL H. HALANG, JR., Filipino, of legal age, single and a resident of Brgy.
Nagba, Cuartero, Capiz after having been duly sworn in accordance with law,
hereby depose and state THAT:

1. I am the registered owner of a certain motor vehicle, more, more

particularly described as follows:

Brand: Kawasaki
Model: CT125A
Color: Black-Red
Engine No.: DYYWKF57828
Chassis No.: MD2A76BYXKWF64824

2. The original body type of the said motor vehicle was “WIHOUT SIDE
CAR”;

3. To have more used of the said motor vehicle, I have decided to change
the body type of the said vehicle form “WITHOUT SIDE CAR” to
“WITH SIDE CAR”

4. The change of body type is not intended for any illegal or unlawful
purpose but solely due to the foregoing reason;

5. As such, I am executing this Affidavit in good faith to attest to the

truthfulness of the foregoing facts.
