AFFIDAVIT OF DRIVER

I, JAN EDISON D. INDING, of legal age, Filipino, married and a resident of

Minaog, Dipolog City, under oath, hereby depose and say, That:

1. I am the driver of a vehicle registered under the name of my brother

Cyrus D. Inding, a certain Brand New Toyota HiAce with plate no.
NB27530 vehicle (vehicle for brevity);

2. On 4 February 2019 at around 3:00 o’clock in the afternoon, I was

driving the vehicle and was about to get out of our garage at our
residence at Minaog, Dipolog City to go to the CityMall, Sta. Filomena,
Dipolog City to buy some groceries when the right side door was
bumped at the gate;

3. Out of tremor, I immediately maneuvered backward but unfortunately

the left side of the car had sideswiped the wall of the garage;

4. The said accident caused damages to the vehicle, to be specific: RH

Front Door and LH Side Panel;

5. The damages sustained by the vehicle entail immediate repairs

particularly for the repaint of RH Front Door and to straighten and
align the LH Side Panel; and

6. I am executing this Affidavit in order to attest to the truth of the

foregoing facts and for all legal intents and purposes this may serve.

IN WITNESS WHEREOF, I have hereunto set my hand and affixed my

signature this 7th day of February 2019 at Dipolog City, Philippines.
