Consent & Undertaking by the Examinee and Parent for the Online Examination-June 2020

under (the District) _______________________ of ______________________ (State). I understand
the present situation and my own requirements. I hereby opt for the online mode of end-semester
examination. I agree to all the terms and conditions as follows:
1. I will use Desktop/Laptop with inbuilt camera/webcam for the examination. I know that exam is
not available on mobile.
2. There will be sufficient light to capture my face.
3. Dress Code is strictly smart casuals. (Ex: Jacket, muffler, caps etc. are not allowed).
4. There will not be any other person in the room during the examination.
5. I have the required infrastructure to appear the examination as under
a. Desktop/Laptop/Macbook(High Sierra+Catalina) with uninterrupted power backup for 3 hrs
b. Intel Dual core or above with Operating System: Windows 7/8.1/10 versions
c. Minimum 4 GB of RAM and Minimum 100 MB of free disk space
d. Internet connectivity: minimum 2 Mbps uninterrupted internet speed
e. Updated Chrome/Firefox web browser
6. I know that appearing mock test is mandatory prior to examination.
7. I will not open any other browser during examination.
8. I agree to undertake the examination in proctored mode. The images / videos will be captured
and monitored on real time basis by the invigilator/proctor.
9. Virtual calculator is provided hence physical calculator is prohibited.
10. I will keep mobile in “Do not Disturb” mode.
11. I shall use blank A4 size paper, wherever necessary, for writing the answers which will be
scanned and uploaded during the examination using mobile camera. I will keep mobile phone in
flight mode. Scanned written answer copies, preferably with Cam Scanner, in pdf format will be
uploaded by me.
12. I will follow all the relevant instructions as used during physical examination.
13. Blocking the camera will lead to malpractice.
14. Biological breaks are permitted only between SAQ and LAQ as specified.
15. Any type of non-compliance will be treated as malpractice (UFM).
16. I will abide by the rules and regulations of the university as notified from time to time with
respect to the examination.
